Abstract

Analisis kebijakan Gerakan Nasional Percepatan Perbaikan Gizi ini bertujuan menganalisis kebijakan (analysis for policy) terkait Peraturan Presiden No. 42 tahun 2013 melalui program 1000 HPK beserta evaluasi dalam memberikan alternatif pemecahan masalah kebijakan yang berlaku. Terdapat kendala dalam gerakan 1000 HPK seperti belum adanya penggalangan komitmen khusus, kurangnya partisipasi masyarakat dalam mendukung program, dan belum maksimalnya dalam melakukan monitoring evaluasi kegiatan terkait program. Mengacu pada SUN Framework, sebagian besar dari 13 intervensi di bidang gizi yang terbukti paling cost effective sudah dilaksanakan, namun hasilnya tidak efektif. Hal ini terutama karena masalah gizi sementara ini dianggap sebagai tanggung jawab sektor kesehatan semata. Abstract

Objective: This study examines public health problems related to the prevention of stunting and anemia through practical education for young girls, especially with the Cooking and Healthy Girls Club program. Content: Stunting cases in Serang Regency are dominated by middle to lower economic communities who have misunderstandings in the pattern of intake for pregnant women. Problems during adolescence, specifically inadequate nutritional intake and suffering from anemia for a long time will have a negative impact on future mothers. The long-term impact can have a big chance of giving birth to a stunted baby. Young people today like to go on strict diets because they want to have a slim body without thinking about their health. Therefore, the understanding of health for prospective mothers should have been taught a lot and have entered the high school level through the Education Authorities. Early education, especially in the adolescent phase, is the best solution in preventing prolonged stunting. Cooking and Healthy Girls Club can be alternative education that is able to answer problems related to nutrition and health of adolescent girls. In this club program there is collaboration with certain parties who are experts in the fields of culinary, nutrition, and youth health. In this case, it is necessary to involve catering management, foodpreneurs, and KSR PMI (Indonesian Red Cross). The development of health-based youth education programs is not enough with the lecture method alone, but the need for practice and skills, especially in fulfilling nutritious and healthy food. The ability to do business and entrepreneurship is currently a dream for young people. The development of a healthy food business is a potential that needs to be developed by adolescents.

Abstract

Objective: This study examines public health problems related to anemia prevention through breakfast education for adolescent girls by developing ProGAS (Program Gizi Anak Sekolah) that have been implemented in previous years into “ProGAS from home” that is determined from their respective homes during the pandemic. Content: Around 22.7% of Indonesian females aged between 14 and 18 years suffer from anemia, a condition that is most commonly caused by micronutrient deficiencies and malnutrition. Based on the survey results obtained from 2016 to 2018, the adolescent anemia rate in Sleman Regency tends not to experience a significant decrease. The prevalence of anemia in adolescent girls is expected to increase during the pandemic. Some of the main factors are the habit of not having breakfast, poor diet, and economic factors. Since schools were closed due to the pandemic, teenagers do not have any access to information and knowledge about nutrition and women’s health. Schools also find it difficult to provide nutrition education outside the school curriculum. In addition, youth-based health education programs are not enough with one-way communication, but also need to practice by providing breakfast as a fulfillment of nutritious and healthy food. ProGAS from home can be an alternative education that can prevent anemia in adolescent girls through habituation to breakfast in their respective homes. ProGAS which is promoted by the Ministry of Education and Culture has previously been able to meet the nutritional needs of students in several schools. In this program, it is necessary to involve local SMEs such as catering or healthy restaurants in providing breakfast. Thus, in addition to achieving the main program, it can also increase income for local SMEs who have been affected by the pandemic.

Abstract

Objective: This study examined the puskesmas (public health center) head's role in mobilizing across sectors in handling the stunting problem, especially in the community working area of ​​the Puskesmas Mlati II. Contents: The head of the puskesmas is a manager and a leader in coordinating every activity carried out at the puskesmas, including handling stunting problems. As much as 70% of the factors for stunting are outside the health sector. Some institutions have budgets that are top-down but not communicated to other cross-sectors. In addition, at the sub-district level, someone has to take on the role of a stunting program manager who can map the problem and then divide tasks into implementation. It requires the expertise of a head of the puskesmas to explain to parties outside of health to be willing and able to play a role in handling stunting in the community. The advocacy ability of a puskesmas head determines the success of the program carried out. Personal closeness is usually more successful in transferring knowledge and fostering a willingness to be involved in problem-solving. Advocacy of social networking integration & mobilization, interpersonal communication, negotiation, and media use to generate public pressure. Therefore, there is a need for a new way of using advocacy communication strategies to deal with the stunting problem, especially during the pandemic. Evaluating and monitoring activities reported at cross-sectoral meetings every three months led by the sub-district head accompanied by the head of the puskesmas to the synergy of stunting prevention activities and programs can be directed with more explicit goals. In addition, advocacy can also be carried out together with local village heads by creating awareness and concern for stunting, such as forming an alliance or the First 1000 Days of Life (HPK) movement for its citizens and others. Tujuan: Penelitian ini mengkaji peran kepala puskesmas dalam menggerakkan lintas sektor untuk menangani masalah stunting, khususnya pada masyarakat di wilayah kerja Puskesmas Mlati II. Isi: Kepala puskesmas merupakan manajer sekaligus leader yang berperan dalam mengkoordinir setiap kegiatan yang dilakukan di Puskesmas, termasuk dalam menangani masalah stunting. Sebesar 70% faktor terjadinya stunting justru adalah di luar sektor kesehatan. Beberapa institusi telah memiliki anggaran yang sifatnya top down, namun tidak dikomunikasikan dengan lintas sektor yang lain. Selain itu, di tingkat kecamatan juga harus ada yang mau mengambil peran sebagai manajer program stunting, karena program tersebut dalam pelaksanaan di lapangan masih terfragmentasi. Oleh karenanya perlu seorang manajer program yang memahami peta masalah dan kemudian membagi tugas agar tidak tumpang tindih dalam pelaksanaan di lapangan, sehingga sasaran bisa mendapatkan manfaat yang optimal. Hal ini tentunya membutuhkan keahlian dari seorang kepala puskesmas untuk dapat menjelaskan kepada pihak di luar kesehatan untuk mau dan mampu berperan dalam penanganan stunting di masyarakat. Kemampuan advokasi seorang kepala puskesmas menentukan keberhasilan program yang dilakukan. Kedekatan secara personal biasanya lebih banyak berhasil dalam transfer pengetahuan yang pada akhirnya mampu menumbuhkan kemauan untuk terlibat dalam penyelesaian masalah. Advokasi penggabungan jejaring sosial & mobilisasi, komunikasi interpersonal, negosiasi, serta penggunaan media untuk menghasilkan tekanan publik. Oleh karena itu perlu adanya cara baru dalam menggunakan strategi komunikasi advokasi untuk menangani masalah stunting, terlebih di masa pandemi. Ini bisa dilakukan dimulai dari proses evaluasi dan monitoring kegiatan yang telah dilaporkan pada pertemuan lintas sektor setiap 3 bulan yang dipimpin oleh camat dengan didampingi kepala puskesmas, sehingga adanya sinergi kegiatan dan program penanggulangan stunting yang bisa diarahkan dengan tujuan yang lebih jelas. Selain itu advokasi juga bisa dilakukan bersama-sama dengan kepala desa setempat dengan menciptakan awareness dan kepedulian terhadap stunting, misalnya dibentuk aliansi atau gerakan 1000 Hari Pertama Kehidupan (HPK) pada warganya, dan lainnya.

Abstract

There are several adolescent girls who have experienced anemia in villages around Puskesmas Mlati II, especially at the junior high school level, 21.03% of a total of 233 students in January 2020, which is before the COVID-19 pandemic. The prevalence of anemia is expected to increase after the pandemic. The main factors are the habit of not eating breakfast, poor diet, and lack of adolescent health services. The poverty factor in the family and the irregular distribution of blood supplement tablets are the drivers of the problem of anemia. As for the prevention of the problem of anemia of adolescent girls in the villages around the Puskesmas Mlati II area, the PKPR and nutritionist plan to establish adolescent posyandu in each hamlet. It is hoped that the initiation of the adolescent posyandu will not only carry out posyandu with five tables, but can also include other additional programs such as educational webinars, online peer education, shared breakfast practices, and permaculture nutrition garden practices. The purpose of this activity is to create an adolescent posyandu as one of the efforts of adolescent girls' health services in preventing anemia. The results of this activity are in the form of a description of the urgency of adolescent posyandu for the health of adolescent girls and the initiation of the adolescent posyandu itself, such as situation analysis of the problem of anemia, planning of program implementation, including the formation of adolescent cadres, and preparing a draft evaluation document.
