Abstract

Background: Cesarean scar pregnancy (CSP) is a rare and potentially life-threatening form of ectopic pregnancy in which the gestational sac implants within the myometrial scar from a previous cesarean section. It is associated with significant risks, including uterine rupture and severe hemorrhage. Case Presentation: We report the case of a 35-year-old multiparous woman (G5P4A1) with two prior cesarean deliveries who presented with persistent vaginal spotting for three weeks, worsening over the last three days. She had undergone uterine curettage for incomplete abortion three weeks prior. A transvaginal ultrasound revealed a hypoechoic mass in the cesarean scar niche, with positive vascularization, consistent with a diagnosis of CSP. Surgical intervention via laparotomy was performed, including evacuation of retained products of conception, uterine resection and reconstruction, bilateral tubectomy, and adhesiolysis. The patient recovered well and was discharged after three days of postoperative care. Conclusion: Early diagnosis and prompt surgical management of cesarean scar pregnancy are essential to prevent severe complications. This case emphasizes the importance of considering CSP in patients with prior cesarean sections presenting with abnormal uterine bleeding after early pregnancy loss.

Abstract

Penelitian ini bertujuan untuk mengevaluasi efektivitas metode alami seperti Shettles Method dan pendekatan berbasis diet serta membandingkannya dengan teknologi modern Preimplantation Genetic Testing (PGT) dalam pemilihan jenis kelamin anak. Penelitian ini merupakan studi sistematik dengan pendekatan kualitatif deskriptif. Populasi penelitian mencakup artikel ilmiah yang diterbitkan dalam 10 tahun terakhir (2014-2024) terkait metode pemilihan jenis kelamin. Sampel penelitian terdiri dari 15 artikel yang memenuhi kriteria inklusi, yang dipilih menggunakan teknik purposive sampling. Data dikumpulkan melalui penelusuran database PubMed dan Google Scholar menggunakan kata kunci spesifik, seperti "Shettles Method," "diet-based gender selection," dan "Preimplantation Genetic Testing." Analisis data dilakukan dengan menggunakan alat Critical Appraisal untuk menilai kualitas studi, dan hasil disintesis berdasarkan efektivitas, etika, dan dampak sosial dari masing-masing metode. Hasil penelitian menunjukkan bahwa Shettles Method dan pendekatan diet memiliki tingkat keberhasilan yang terbatas, sedangkan PGT menawarkan akurasi tinggi hingga 99% tetapi menghadirkan tantangan etis, seperti ketidaksetaraan gender dan aksesibilitas teknologi. Penelitian ini menyarankan perlunya panduan yang lebih jelas serta edukasi masyarakat mengenai risiko dan implikasi metode pemilihan jenis kelamin.

Abstract

Background: Female reproductive organs' congenital abnormalities consist of abnormalities in the hymen, vagina, cervix, uterus, and others1. These abnormalities can affect women's ability to menstruate, sexual relations, reproduction, and psychological conditions2. Management of this abnormality depends on the defects of the organs involved and the expected improvement in function. This research was conducted because of the limitation of these cases.Objective: This study aims to determine the quality of life after surgery in women with congenital abnormalities of the reproductive organs.Methods: This study used a prospective study with a descriptive observational cross-sectional method. The population was patients at the Obstetrics Gynecology Polyclinic of Dr. Sardjito Hospital Yogyakarta in 2022 who had congenital disorders of the reproductive organs, aged at least 17 years old, and had undergone surgery. Participants filled out the FSFI and sociodemographic questionnaire.Results: There were 15 participants with 3 types of congenital abnormalities, namely 2 hymenal disorders (microperforate hymen and cribiform hymen), 9 vaginal abnormalities (vaginal agenesis and cervicovaginal agenesis), and 4 uterine and vaginal abnormalities (HWW syndrome). After surgery, 13 people experienced an improvement in abdominal pain; 9 people with primary amenorrhoea were able to menstruate; and 8 married people were able to have sex after surgery. For sexual function, 62.5% of participants had good FSFI scores.Conclusion: Surgery can provide a good function for abdominal pain, menstruation, and sexual relations.
