Abstract

Otitis eksterna maligna (OEM) adalah infeksi progresif pada saluran pendengaran eksternal dan dasar tengkorak yang sering disebabkan oleh Pseudomonas aeruginosa. Kondisi ini umumnya menyerang penderita diabetes melitus, lansia, dan individu dengan gangguan sistem kekebalan tubuh. Meskipun jarang, OEM merupakan kondisi fatal yang memerlukan diagnosis dini dan tatalaksana yang tepat untuk mencegah komplikasi serius, termasuk parese nervus fasialis. Referat ini membahas definisi, epidemiologi, etiologi, patofisiologi, manifestasi klinis, penegakan diagnosis, dan tatalaksana OEM, serta melaporkan satu kasus Otitis Eksterna Maligna. Penegakan diagnosis melibatkan anamnesis, pemeriksaan fisik, laboratorium (LED, CRP), mikrobiologi (kultur), dan pencitraan radiografi (CT-scan, MRI, PET-CT). Tatalaksana meliputi medikamentosa (antibiotik, antijamur), pembedahan (debridemen, mastoidektomi), dan terapi komplementer (oksigen hiperbarik). Pemahaman komprehensif tentang OEM sangat penting bagi ahli THT-BKL untuk memberikan penanganan yang akurat dan efektif.

Abstract

Hemangioma is a benign vascular tumor that commonly occurs in children, but its presentation on the nasal dorsum is rare, especially among adolescents. This case report describes a 16-year-old male patient with a progressively enlarging, painful nasal mass over seven months. Clinical workup including CT-scan, angiography, and FNAB revealed a vascular lesion suggestive of hemangioma. The patient underwent pre-operative embolization followed by surgical excision. Histopathological and immunohistochemical evaluations confirmed the diagnosis of intraosseous capillary hemangioma. Postoperative follow-up showed good aesthetic and functional outcomes with no recurrence. This report highlights the importance of comprehensive diagnostic and surgical approaches for managing rare hemangioma of the nasal dorsum to minimize complications and preserve facial structure.
