Abstract

AKI worldwide according to WHO in 2018, namely 108,300 In Indonesia, AKI cases in 2020, namely 6,856. In Central Java, MMR in 2021 is 1,011 with Tegal Regency at 29.78% and IMR 48 cases. Data at the Jatibogor Health Center for Chronic Energy Deficiency (KEK) cases amounted to 16.7% of a total of 840 pregnant women. Pregnant women with CED will certainly experience various health problems. The health of pregnant women is very important because it can affect the health of the baby they contain. KEK is a disease that has several risk factors and adverse effects on the mother and baby. The strategy for achieving government policy is through improving Indonesia's health in accordance with the Sustainable Development Goals (SDG) by forming quality human beings whose one of the achievements is meeting the food and nutrition needs of each individual. Collaborate with health workers to provide KIE regarding KEK and the factors that influence it and how to overcome them. One of them is counseling by spreading messages, instilling confidence. The purpose of this case study was to carry out midwifery care for pregnant women, birth mothers, postpartum women, and newborns in a comprehensive manner using midwifery care management according to Varney and documentation using the SOAP method. The object of this case is Mrs. H G1P0A0, 27 years old with normal pregnancy, childbirth and postpartum. This case study was carried out in October 2022 in the working area of ​​the Jatibogor Health Center. The care is described in its entirety, starting from the patient's TM III pregnancy (39 weeks to 41 weeks) and normal postpartum (9 hours postpartum to 40 days postpartum). The processed results are Comprehensive Midwifery Care for Ny. H since the age of 39 weeks, during labor until puerperium 40 days postpartum. The compilation concludes that the gestation period is normal, delivery by SC, BBL and postpartum are normal.

Abstract

Data at the Jatibogor Health Center for obesity cases total of 16 pregnant women. Obesity is a condition of conflict between body weight and height due to excess fat tissue in the body. The incidence rate of overweight or obesity is increasing over time, even one of which became a global pandemic with a prevalence of 2.1 billion in 2013, an increase from 857 million cases in 1980. Obesity can be assessed from the body mass index (BMI), which World Health Organization (WHO) classifies obesity as BMI≥ 30 kg m2. Obesity generally occurs in pregnancy, especially pregnancies that occur over 35 years. This state of obesity is a high risk obstetrics because it can increase the risk of morbidity and mortality of the mother and fetus. Complications that can occur in pregnant women with a risk of obesity in pregnant women are an increase in gestational diabetes hypertension, spontaneous abortion and postpartum hemorrhage. In the fetus it can increase the risk of stillbirth in the antepartum period, intrapartum complications such as shoulder dystocia, macrosomia and increase the risk of fetal defects such as neural tube defects, spina bifida, congenital heart disease and omphalocele. Macrosomia is a condition in which a baby is born with a larger size or a body weight of 4000 grams, this is one of the complications of childbirth which causes increased morbidity and mortality in pregnancy because it causes many complications in childbirth.

Abstract

In 2022, Pangkah Public Health Center in Tegal Regency recorded 2 cases of maternal deaths, attributed to hemorrhage and umbilical cord embolism. Data obtained from the health center in 2023 revealed a total of 874 pregnant women, with 475 categorized as high-risk. Risk factors included maternal age <20 years (17 cases), maternal age >35 years (133 cases), interpregnancy interval <2 years (31 cases), nutritional problems (109 cases), anemia (50 cases), presence of more than 5 children (17 cases), and others (118 cases). Among high-risk pregnancies, women aged >35 years and those falling into the "others" category were the two most prevalent groups. The general objective of this case study is to gain insights and real-life experiences in providing comprehensive midwifery care to Mrs. D at Pangkah Public Health Center in 2023, focusing on high-risk grandemultipara and women aged >35 years, utilizing the 7-step Varney midwifery care management and SOAP development data. This research adopts a case study approach, with Mrs. D, a 36-year-old grandemultipara with advanced maternal age, as the subject. Comprehensive care is provided throughout the antenatal, intrapartum, postpartum, and neonatal periods, including the application of Postnatal Massage therapy. From the data collected, it is concluded that the subject experienced numbness in her hands upon waking up from 38 weeks of gestation onwards. The patient perceives her pregnancy as highly risky due to being a grandemultipara and advanced maternal age, necessitating close monitoring by healthcare providers.
