Abstract

Tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis. The tuberculosis (TB) detection methods used are the GeneXpert method and the Ziehl-Neelsen method. GeneXpert identifies the rpoB hotspot gene bacteria in Mycobacterium tuberculosis by analyzing deoxyribonucleic acid (DNA), while the TB detection method with Ziehl-Neelsen staining is a microscopic examination used to detect acid-fast bacilli (AFB), namely Mycobacterium tuberculosis bacteria. The purpose of this study was to determine the sensitivity, specificity and positive predictive value of the nucleic acid amplification test (GeneXpert) using sputum samples in patients with suspected pulmonary tuberculosis and compare it with microscopic examination (Ziehl-Neelsen staining) of Acid-Fast Bacilli (AFB). The method used is simple random sampling which explains the comparison between the variables of the GeneXpert and Ziehl-Neelsen examination results in pulmonary tuberculosis patients using a cross-sectional approach, namely where data is obtained from the pulmonary TB population at Hospital X Yogyakarta from 2021-2024 according to the variables studied. Based on the TCM results which are greater than the BTA results, these results indicate a significant difference between the GeneXpert method and the Ziehl-Neelsen method in terms of examination results. The results obtained from the examination using the TCM/GeneXpert and Ziehl-Neelsen methods show a higher detection rate of GeneXpert than Ziehl-Neelsen in identifying TB cases.

Abstract

Diare masih menjadi salah satu masalah kesehatan global yang serius dan merupakan penyebab utama kematian pada anak di bawah usia lima tahun, terutama di negara berkembang. Shigella boydii ATCC 9207 dan Salmonella typhimurium ATCC 14028 merupakan bakteri Gram negatif utama penyebab infeksi diare. Meningkatnya kasus resistensi antibiotik mendorong upaya pencarian agen antibakteri alternatif berbasis bahan alami, salah satunya bawang putih tunggal (Allium sativum L.). Penelitian ini bertujuan untuk mengetahui aktivitas antibakteri ekstrak etanol bawang putih tunggal terhadap Salmonella typhimurium dan Shigella boydii. Penelitian eksperimental laboratorium ini menggunakan metode difusi cakram (Kirby-Bauer) dengan ekstrak yang diperoleh melalui proses maserasi menggunakan etanol 96%. Uji antibakteri dilakukan terhadap Salmonella typhimurium ATCC 14028 dan Shigella boydii ATCC 9207 pada konsentrasi ekstrak 50%, 70%, dan 90%, dengan kontrol positif berupa kloramfenikol dan kontrol negatif berupa aquades steril, masing-masing dilakukan lima kali ulangan. Data dianalisis menggunakan uji Shapiro-Wilk, Kruskal-Wallis, dan Mann-Whitney. Hasil penelitian menunjukkan bahwa ekstrak etanol bawang putih tunggal tidak menghasilkan zona hambat terhadap Salmonella typhimurium pada seluruh konsentrasi, sedangkan terhadap Shigella boydii menunjukkan aktivitas antibakteri lemah pada konsentrasi 70% dan 90%, dengan rata-rata zona hambat masing-masing sebesar 1,2 mm dan 1,7 mm. Kesimpulannya, ekstrak etanol bawang putih tunggal berpotensi sebagai agen antibakteri alami terhadap Shigella boydii ATCC 9207, namun tidak efektif terhadap Salmonella typhimurium ATCC 14028.

Abstract

Pulmonary tuberculosis (TB) continues to pose a significant worldwide health concern, particularly when associated with complications. Delays in diagnosis and identification of drug resistance in these cases can increase morbidity and mortality. This descriptive analytical study using cross-sectional methodology aims to explain the results of GeneXpert examination, a rapid molecular diagnostic instrument, in patients with pulmonary tuberculosis with complications at Panti Rapih Hospital, Yogyakarta. In 2024, secondary data collection using a total sampling strategy, which included 660 patients with pulmonary and extrapulmonary tuberculosis, resulted in 132 cases of pulmonary tuberculosis and 32 samples that met the inclusion criteria. The findings showed that the sample was dominated by men (56.3%) of productive age (75%), with pleural effusion as the most common complication. The main GeneXpert finding was that more than half of the samples (59.4%) showed "MTB Detected Low" results that were sensitive to Rifampicin. This profile confirms the urgency of using GeneXpert as a rapid diagnostic tool to determine appropriate treatment strategies early, particularly in complex patients at high risk of therapy failure.
