<![CDATA[ABSTRACT Consent for medical action (Informed Consent) is an agreement given by the patient or closest family after receiving a complete explanation of the medical or dental action to be performed on the patient. The purpose of this study was to determine the completeness of filling out the informed consent form for caesarean section as legal evidence at Setia Mitra Hospital, South Jakarta Period 2020. This research method uses descriptive research, data collection uses observation sheets and interview sheets. The population in this study were 121 medical record files for Caesarean section patients and a sample of 55 informed consent forms was obtained using simple random sampling technique. The results showed that at Setia Mitra Hospital, South Jakarta, there was no SOP for filling out informed consent, and the implementation of the legal aspects of informed consent had not been fully implemented because the informed consent form at Setia Mitra Hospital, South Jakarta, specifically for the caesarean section, had never been used as evidence. law. When filling out the informed consent form, it is known that the percentage of completeness that reaches 100% is medical record number, patient name, gender, place date of birth, diagnosis (WDDD), basic diagnosis, medical action, procedures, goals, TTD doctor's clear name surgery, TTD Witness Hospital's clear name. Based on this, it is necessary to increase the completeness of filling out the informed consent form for caesarean section, especially on the type of information such as indications of action, risks, complications, prognosis, alternatives and author authentication such as TTD and clear names of patient witnesse. ABSTRAKLatar Belakang : Persetujuan tindakan medis (Informed Consent) adalah persetujuan yang diberikan oleh pasien atau keluarga terdekat setelah mendapatkan penjelasan secara lengkap mengenai tindakan kedokteran atau dokter gigi yang akan dilakukan terhadap pasien. Tujuan penelitian ini adalah untuk mengetahui kelengkapan pengisian formular informed consent tindakan section caesaria sebagai alat bukti hukum di Rumah Sakit Setia Mitra Jakarta Selatan Periode 2020. Metode penelitian ini menggunakan jenis penelitian deskriptif, pengumpulan data menggunakan lembar observasi dan lembar wawancara. Populasi dalam penelitian ini adalah sebanyak 121 berkas rekam medis pasien Section caesaria dan didapatkan sampel sebanyak 55 formulir informed consent dengan menggunakan teknik simple random sampling. Hasil penelitian menunjukan bahwa di Rumah Sakit Setia Mitra Jakarta Selatan belum ada SOP pengisian informed consent, dan pelaksanaan aspek hukum informed consent belum terlaksana sepenuhnya karena formulir informed consent di Rumah Sakit Setia Mitra Jakarta Selatan khusus nya pada tindakan section caesaria belum pernah dijadikan sebagai alat bukti hukum. Pada pengisian formulir informed consent diketahui bahwa presentase kelengkapan yang mencapai 100% yaitu nomor rekam medis, nama pasien,jenis kelamin, tempat tanggal lahir, diagnosis (WDDD), dasar diagnosa, tindakan kedokteran, tata cara, tujuan, TTD nama jelas dokter bedah, TTD Nama jelas Saksi Rumah Sakit.
