Abstract

Effective and efficient drug management is a crucial element in ensuring the availability of medications, especially for chronic diseases such as hypertension. This study aims to evaluate the management system of antihypertensive drugs during the planning and procurement stages at the pharmaceutical warehouse of Varia Pharmacy, located on Gatot Subroto Street (Gatsu). The main focus of this research is to identify the extent to which drug needs planning and procurement systems are appropriately implemented to support optimal pharmaceutical services. This study uses a descriptive qualitative method through direct observation, interviews with warehouse staff and pharmacists, as well as document reviews related to drug procurement and planning. The evaluation results indicate that the planning process for antihypertensive drugs is not entirely based on actual usage data but still relies on routine estimations that do not consider fluctuations in patient demand. Furthermore, there are challenges in procurement, such as delays in supplies from distributors and lack of synchronization between service units and the pharmaceutical warehouse. These issues have resulted in stockouts during certain periods, potentially disrupting the continuity of hypertension treatment. Therefore, it is recommended to implement a planning system based on historical data and to digitize the logistics process to improve efficiency. This research provides valuable insights for pharmacy managers to enhance drug management quality and patient service

Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease with high prevalence and significant public health impact. This study aimed to analyze the association of IL-6 gene polymorphism (−174G>C) with serum IL-6 levels and the effectiveness of standard therapy assessed through HbA1c and Random Blood Glucose (RBG) in T2DM patients. A cross-sectional design was employed, involving 40 patients at Imelda Hospital. The polymorphism was examined using Polymerase Chain Reaction (PCR), serum IL-6 levels were measured with Enzyme-Linked Immunosorbent Assay (ELISA), while HbA1c and RBG were determined through standard laboratory analysis. The results indicated that IL-6 genotypes (GG, GC, CC) were not significantly associated with serum IL-6 levels, HbA1c, or RBG. In contrast, type of therapy showed a significant relationship with HbA1c, where patients receiving combination therapy achieved better glycemic control compared to those on monotherapy. These findings highlight that therapeutic regimen remains the primary determinant of glycemic control, whereas the role of IL-6 polymorphism in this study population appears limited. Further studies with larger sample sizes and broader genetic analyses are required to strengthen the application of precision medicine for diabetes management in Indonesia.

Abstract

Sistem komunikasi nirkabel masa depan sedang berkembang menuju infrastruktur 6G yang sangat heterogen dengan mengintegrasikan lingkungan terestrial, udara, komputasi tepi (edge), dan radio cerdas. Tinjauan pustaka sistematis ringkas ini mensintesis gagasan-gagasan utama optimisasi berbasis kecerdasan buatan (artificial intelligence/AI) untuk jaringan nirkabel masa depan. Tinjauan ini berfokus pada tujuh dimensi dominan: alokasi sumber daya berbasis AI, pembelajaran terfederasi dan terdistribusi, kecerdasan tepi (edge intelligence), permukaan cerdas yang dapat dikonfigurasi ulang (reconfigurable intelligent surfaces), pemotongan jaringan (network slicing) dan O-RAN, kembaran digital (digital twins) dan komunikasi semantik, serta penginderaan dan komunikasi terintegrasi berbasis UAV. Hasil analisis menunjukkan bahwa deep reinforcement learning telah diadopsi secara luas untuk kontrol sumber daya radio yang dinamis, sementara pembelajaran terfederasi menjadi elemen sentral dalam kecerdasan terdistribusi yang menjaga privasi. Studi-studi terbaru juga mengindikasikan adanya konvergensi antara AI generatif, komunikasi semantik, dan pengelolaan jaringan otonom. Kendati demikian, masih terdapat tantangan yang belum terpecahkan terkait standardisasi tolok ukur (benchmark), generalisasi model, beban komputasi (computational overhead), privasi, dan kesiapan implementasi. Artikel ini menyajikan taksonomi ringkas serta arah penelitian masa depan untuk optimisasi jaringan nirkabel cerdas.

Abstract

Hypoalbuminemia is a common complication in patients with Chronic Kidney Disease (CKD) and is associated with increased morbidity, mortality, and length of hospital stay. Human albumin therapy is effective but costly; therefore, pharmacoeconomic evaluation of alternative therapies based on snakehead fish (*Channa striata*) extract is warranted. This study aimed to evaluate the cost-effectiveness and clinical outcomes of Channa as an adjuvant to 20% Human Albumin therapy in CKD patients with hypoalbuminemia at Imelda Pekerja Indonesia General Hospital, Medan. A descriptive observational study with retrospective data collection was conducted involving 35 patients who were divided into three treatment groups: Channa product, 20% Human Albumin, and a combination of both therapies. Effectiveness was assessed based on the increase in serum albumin levels (Δ albumin, g/dL), while costs were calculated based on direct costs of albumin therapy. Cost-effectiveness analysis was performed by calculating the Average Cost-Effectiveness Ratio (ACER) and Incremental Cost-Effectiveness Ratio (ICER). The results showed that the total direct cost of albumin therapy was IDR 57,970,200, with the lowest cost observed in the Channa product group. The Channa product group had the lowest ACER (IDR 990,889 per g/dL), and the ICER analysis indicated that Channa was a dominant strategy, being both less costly and more effective. It was concluded that Channa therapy was the most cost-effective option for correcting hypoalbuminemia in CKD patients, while 20% Human Albumin remained important for severe conditions requiring rapid correction.

Abstract

Hypoalbuminemia is a complication that is often found in ileus patients and is associated with an increased risk of complications, length of hospitalization, and the burden of treatment costs. Human albumin therapy is effective but high-cost, so pharmacoeconomic studies are needed on alternative therapies based on snakehead fish extract (Channa striata). This study aims to evaluate the cost-effectiveness and clinical outcomes of the use of Channa products as an adjuvant in 20% Human Albumin therapy in ileus patients with hypoalbuminemia at Imelda Pekerja Indonesia Hospital Medan. The study used an observational analytical design with retrospective data collection of 49 patients who were grouped into three therapy groups: Channa single, Human Albumin 20% single, and a combination of both. Effectiveness is measured using length of stay (LOS) as a proxy for therapy success, while cost is calculated from the direct cost of treatment. Cost effectiveness analysis was carried out through the calculation of Average Cost-Effectiveness Ratio (ACER) and Incremental Cost-Effectiveness Ratio (ICER). The results showed the lowest average direct cost in the single Channa group (IDR 10,155,201) and the highest in the single 20% Human Albumin (IDR 39,806,166). The lowest ACER value was for the single Channa group (Rp 1,846,400 per day of treatment), and in the ICER calculation, the combination group was dominant over the 20% single Human Albumin and the single Channa group was dominant against the other two groups. It was concluded that Channa-based therapy is the most cost-effective option for the correction of hypoalbuminemia in ileus patients, while Human Albumin 20% remains important in severe conditions that require rapid correction prior to surgical procedures.
