Abstract

The increasing burden of infectious diseases such as Tuberculosis (TB) and non-communicable diseases such as Diabetes Mellitus (DM) places significant pressure on the National Health Insurance (JKN) financing system. This study aims to analyze the healthcare financing patterns of TB and DM using BPJS Kesehatan secondary data from 2022 through a clustering analysis approach. This quantitative study employs BPJS Kesehatan Sample Data, covering 34 provinces in Indonesia. Data processing was performed using Stata 14.2 for case proportion calculations and RStudio for cluster analysis with the K-Means algorithm. The optimal number of clusters was determined using the Gap Statistic and Within-Cluster Sum of Squares (WCSS) methods. The results indicate three regional clusters: a moderate-burden cluster (Sumatra), a high-burden cluster (Java), and a low-burden cluster (other provinces).

Abstract

ABSTRACT Violence against healthcare workers in hospitals has become a global concern. This violence often occurs in service areas, both verbal and physical, and can impact the physical and mental health of healthcare workers. Nurdin Hamzah Regional Hospital, as a referral hospital in Tanjung Jabung Timur District, does not yet have an effective policy to address violence against healthcare workers. This study aims to evaluate the implementation of the violence handling policy for healthcare workers at this hospital.This study uses a qualitative approach with a case study method. Data were collected through in-depth interviews with 10 informants, including the hospital director, specialist doctors, members of the violence handling committee, nurses, administrative staff, and security officers. Other data collection techniques include observation and documentation. Data analysis was conducted using thematic analysis with the assistance of NVivo software.The study found that although the violence handling policy has been implemented, there are gaps in communication between hospital units, especially in units with a lower frequency of violence incidents. Additionally, the reporting system for violence is still suboptimal due to psychological barriers from healthcare workers who are concerned about worsening relationships with patients’ families. However, the presence of the healthcare protection committee and a clear reporting system provides support for the implementation of the policy. The implementation of the violence handling policy for healthcare workers at Nurdin Hamzah Regional Hospital is underway, but improvements are needed in communication between units, the reporting system, and increased psychological support for victims. It is recommended that the hospital enhance training and policy socialization and improve the reporting system to better protect healthcare workers from violence. Keywords:  Policy, Violence Against Healthcare Workers, Hospital,  Reporting System.  ABSTRAK Kekerasan terhadap tenaga kesehatan di rumah sakit semakin menjadi perhatian global. Insiden kekerasan verbal maupun fisik sering kali terjadi di ruang pelayanan, yang dapat berdampak pada kesehatan fisik dan mental tenaga kesehatan. RSUD Nurdin Hamzah, sebagai salah satu rumah sakit rujukan di Kabupaten Tanjung Jabung Timur, belum memiliki kebijakan yang efektif untuk menangani kekerasan terhadap tenaga kesehatan. Penelitian ini bertujuan untuk mengevaluasi implementasi kebijakan penanganan kekerasan terhadap tenaga kesehatan di rumah sakit ini. Penelitian ini menggunakan pendekatan kualitatif dengan metode studi kasus. Data dikumpulkan melalui wawancara mendalam dengan 10 informan yang terdiri dari direktur rumah sakit, dokter spesialis, anggota komite penanganan kekerasan, perawat, petugas administrasi, dan satpam. Teknik pengumpulan data lainnya adalah observasi dan dokumentasi. Analisis data dilakukan menggunakan pendekatan tematik dengan bantuan perangkat lunak NVivo. Hasil penelitian menunjukkan bahwa meskipun kebijakan penanganan kekerasan sudah diterapkan, terdapat kesenjangan dalam komunikasi antar unit rumah sakit, terutama di unit dengan frekuensi insiden kekerasan yang lebih rendah. Selain itu, sistem pelaporan kekerasan masih tidak optimal karena hambatan psikologis dari tenaga kesehatan yang khawatir akan memperburuk hubungan dengan keluarga pasien. Meskipun demikian, adanya komite perlindungan tenaga kesehatan dan sistem pelaporan yang jelas memberikan dukungan terhadap implementasi kebijakan. Implementasi kebijakan penanganan kekerasan terhadap tenaga kesehatan di RSUD Nurdin Hamzah sudah berjalan, namun perlu ada perbaikan dalam hal komunikasi antar unit, sistem pelaporan, dan peningkatan dukungan psikologis bagi korban. Disarankan agar rumah sakit meningkatkan pelatihan dan sosialisasi kebijakan serta memperbaiki sistem pelaporan untuk melindungi tenaga kesehatan dari kekerasan. Kata Kunci: Kebijakan, Kekerasan Terhadap Tenaga Kesehatan, Rumah Sakit, Sistem Pelaporan.
