<![CDATA[ABSTRAK Tujuan dari Asuhan Kebidanan Berkesinambungan yaitu agar dapat melakukan asuhan kebidanan yang komprehensif terhadap ibu hamil mulai dari masa kehamilan sampai dengan KB sesuai standart asuhan menggunakan pendokumentasian SOAP melalui pendekatan managemen kebidanan. Hasil dalam pelaksanaan asuhan secara berkesinambungan dari masa kehamilan sampai dengan masa KB terhadap Ny. Y G1P0A0 usia 25 tahun mulai dari usia kehamilan 35 minggu yang dilaksanakan pada tanggal 2 Mei 2025 sampai dengan 23 Mei 2025. Pada kunjungan kehamilan didapatkan keluhan pada ibu yaitu ibu merasa lelah setiap pulang kerja, kaki kram dan ngilu, dan perut tegang dengan keadaan umum ibu baik. Pada Kunjungan Neonatus tidak ada keluhan dan masalah pada bayi. Pada masa nifas Ny. N dilakukan 4 kali dalam kunjungan pertama ibu merasakan perut masih nyeri,ASI sudah keluar namun masih sedikit, pada kunjungan kedua ibu mengalami gangguan pola tidur yang tidak teratur dan ASI belum lancar, pada kunjungan ketiga ibu ASI sudah lancar dan ibu meminta bidan menjelaskan macammacam alat kontrasepsi dan pada kunjungan berikutnya ibu sudah tidak mengalami keluhan dan meminta bidan menjelaskan ulang kontrasepsi hormonal. TTV dan TFU ibu dalam batas normal, rasa nyeri sudah teratasi dan ASI Eksklusif tetap berjalan. Bayi sehat tidak ada komplikasi apapun dan tetap mendapatkan ASI Eksklusif. Ibu menggunakan KB suntik 3 bulan. The results in the continuous implementation of care from pregnancy to birth control for Mrs. Y G1P0A0 aged 25 years starting from 35 weeks of pregnancy which was carried out on Mei 2 2025 to Mei 23 2025. During the puerperium Mrs. Y was done 4 times in the first visit the mother felt the stomach still hurt, the milk had come out but still a little, at the second visit the mother had impaired irregular sleep patterns and breast milk was not smooth, at the third visit the mother breastfed was smooth and the mother asked the midwife to explain the various contraceptives and at the next visit the mother had no complaints and asked the midwife to re-explain hormonal contraception. The mother's TTV and TFU are within normal limits, the pain has been resolved and exclusive breastfeeding continues. Healthy babies do not have any complications and still get exclusive breastfeeding. Mother uses injectable birth control for 3 months.]]>
