Abstract

Angka kematian bayi masih menjadi indikator penting derajat kesehatan masyarakat, dengan keterlambatan pengenalan tanda bahaya neonatal dan praktik perawatan bayi yang kurang tepat berkontribusi terhadap risiko komplikasi termasuk aspirasi. Kegiatan pengabdian masyarakat ini bertujuan meningkatkan pengetahuan ibu mengenai tanda bahaya bayi baru lahir serta keterampilan menyendawakan bayi pada ibu peserta posyandu di wilayah kerja Puskesmas Tanon I. Kegiatan dilaksanakan selama enam hari melalui pendekatan partisipatif yang meliputi observasi, wawancara, telaah data sekunder, penyuluhan interaktif, pelatihan praktik, dan penyediaan media edukasi poster. Hasil kegiatan menunjukkan respons positif peserta, peningkatan pemahaman tanda bahaya bayi, serta kemampuan ibu mempraktikkan teknik penanganan awal tersedak dan cara menyendawakan bayi setelah menyusui. Selain itu, media edukasi yang dihasilkan dapat dimanfaatkan secara berkelanjutan oleh tenaga kesehatan dan kader. Kegiatan ini menunjukkan bahwa intervensi edukasi dan pelatihan berbasis komunitas dapat memperkuat praktik perawatan bayi di rumah dan mendukung upaya pencegahan komplikasi serta penurunan kematian bayi.

Abstract

Abstract:  The increasing elderly population requires community-based health services capable of providing promotive and preventive monitoring. The target community had no elderly integrated health post and local health volunteers lacked basic health examination skills, resulting in suboptimal elderly care. This community service aimed to facilitate the establishment of an elderly integrated health post through capacity strengthening of community health volunteers. The program was implemented using a community service learning approach consisting of coordination, organizational formation, training on basic health examination, elderly exercise training, and mentoring of the first elderly health post implementation. Monitoring and evaluation were conducted through observation and feedback discussions. The results demonstrated the successful establishment of the elderly health post structure, improved volunteer skills in vital sign assessment, and the implementation of the first elderly health post attended by 56 older adults. Volunteers showed increased practical skills and confidence in providing services, while community participation indicated positive acceptance of the program. Mentored establishment of an elderly health post through volunteer capacity strengthening supports the availability of sustainable community-based elderly health services.Abstrak: Peningkatan jumlah lansia menuntut tersedianya pelayanan kesehatan berbasis komunitas yang mampu melakukan pemantauan kesehatan secara promotif dan preventif. Wilayah mitra belum memiliki posyandu lansia serta kader belum memiliki keterampilan pemeriksaan kesehatan dasar sehingga pelayanan lansia belum optimal. Kegiatan pengabdian ini bertujuan memfasilitasi pembentukan posyandu lansia melalui penguatan kapasitas kader kesehatan. Metode pelaksanaan menggunakan pendekatan KKN dengan tahapan koordinasi, pembentukan struktur organisasi, pelatihan pemeriksaan kesehatan dasar, pelatihan senam lansia, serta pendampingan pelaksanaan posyandu lansia perdana. Hasil kegiatan menunjukkan terbentuknya struktur posyandu lansia, peningkatan kemampuan kader dalam melakukan pemeriksaan tanda vital, serta terselenggaranya posyandu lansia perdana dengan partisipasi 56 lansia. Kader menunjukkan peningkatan keterampilan praktis dan kepercayaan diri dalam pelayanan lansia, sementara masyarakat menunjukkan respons positif terhadap layanan yang terbentuk. Pendampingan pembentukan posyandu lansia melalui penguatan kapasitas kader terbukti mendukung tersedianya layanan kesehatan lansia berbasis masyarakat yang berpotensi berkelanjutan.

Abstract

Near-fatal asthma (NFA) is a life-threatening manifestation of acute severe asthma characterized by hypercapnia, respiratory acidosis, altered consciousness, and the need for invasive mechanical ventilation. Early recognition and timely airway stabilization are essential to prevent respiratory arrest and mortality, particularly in resource-limited settings. A 41-year-old woman with a history of asthma was brought to the emergency department with decreased consciousness and severe respiratory distress. On arrival, her Glasgow Coma Scale score was 5, respiratory rate was 35 breaths per minute, and oxygen saturation was 46% on room air. Arterial blood gas analysis demonstrated life-threatening hypercapnic respiratory failure (pH 6.954; PaCO₂ 95.4 mmHg; P/F ratio 87). Due to worsening respiratory failure and decreased level of consciousness, the patient underwent immediate endotracheal intubation and invasive mechanical ventilation and was admitted to the intensive care unit. She received systemic corticosteroids, repeated bronchodilator therapy, aminophylline infusion, and broad-spectrum antibiotics. Lung-protective ventilatory strategies with prolonged expiratory time were applied. The patient improved rapidly and was successfully extubated on the second day of mechanical ventilation, with subsequent discharge in stable condition. Appropriate ventilatory strategies combined with guideline-based pharmacological therapy are essential to improve outcomes in patients with near-fatal asthma presenting with severe hypercapnic respiratory failure, even in resource-limited settings.
