Abstract

Background: RSUD Sultan Sulaiman in Serdang Bedagai Regency faces challenges in implementing the Hospital Information System (SIRS), particularly in the medical record unit. Although several departments have started implementing SIRS, manual record-keeping is still practiced in the medical record unit, leading to delays in patient data flow and a lack of coordination between units. Research Objectives: This study aims to analyze the implementation of SIRS at RSUD Sultan Sulaiman, focusing on the medical record unit, and to identify the factors that hinder the system's implementation. This research also provides recommendations to improve the efficiency of medical data management. Research Methodology: This research uses a qualitative approach with descriptive analysis methods. Data was collected through interviews with pharmacy officers, head of the pharmacy department, head of the medical record unit, and direct field observations. Thematic analysis was used to identify the factors hindering and benefiting the implementation of SIRS. Research Results: The implementation of SIRS has expedited administrative processes and improved coordination between units. However, challenges related to human resources, training, and inadequate hardware continue to hinder optimal implementation. Conclusion: Intensive efforts in training, provision of adequate hardware, and regular system evaluation are needed to support the optimal implementation of SIRS, which is expected to improve the efficiency of medical services at RSUD Sultan Sulaiman.

Abstract

Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain significant challenges in Indonesian healthcare. The speed of medical intervention, or response time, in the PONEK unit is a critical factor in saving the lives of mothers and newborns. As a 24-hour PONEK service provider, Sultan Sulaiman Regional General Hospital must ensure that emergency response times meet established standards to prevent fatal complications. This study aims to analyze the implementation of response time in the Comprehensive Emergency Obstetric Neonatal Care (PONEK) unit at Sultan Sulaiman Regional General Hospital in 2025, examined through aspects of human resource availability, competence, shift distribution, and specialist coordination. This research employs a descriptive qualitative method. Data collection was conducted through in-depth interviews with key informants (ER doctors, PONEK midwives, nurses) and triangulation informants (Head of Medical Services, Head of PONEK). Data analysis was performed thematically to provide an in-depth overview of the supporting and inhibiting factors of response time. The study identified that response time is influenced by the availability of 24-hour on-call specialists, the number of team members on each shift, and the effectiveness of coordination between the ER and the PONEK unit. Identified obstacles include unbalanced workloads at certain times and the need for regular competency training updates for the PONEK team.

Abstract

Post-claim audits conducted by BPJS Kesehatan serve as a supervisory mechanism to ensure accountability in the management of the National Health Insurance (JKN) funds. However, audit practices that result in claim corrections or deductions may cause financial losses to hospitals and raise concerns regarding procedural and distributive justice. This study aims to analyze the implications of post-claim audits on hospital financial losses from the perspective of accountability and the principles of justice at RSU Grandmed. This research employed a mixed-method approach with a cross-sectional design involving 97 respondents selected through purposive sampling. Quantitative data were analyzed using multiple linear regression, examining variables such as transparency of the claim process, auditor accountability, tariff adequacy, timeliness of payment, and allocation of claim funds. The findings indicate that, partially, transparency of the claim process, tariff adequacy, timeliness of payment, and allocation of claim funds significantly influence the hospital’s financial condition, while auditor accountability was not significant in the bivariate analysis. However, simultaneously all variables demonstrated a significant effect on hospital financial losses and financial sustainability (R² = 0.693). These findings demonstrate that post-claim audits are not merely administrative mechanisms but have direct implications for hospital financial stability. Therefore, audit mechanisms must be implemented in a transparent, consistent, objective, and justice-oriented manner to maintain a balance between public accountability and the sustainability of health service delivery.Keywords: Indonesia Health Social Security Agency; post-claim audit; accountability; principles of justice; hospital financial loss.
