Abstract

Electronic Medical Records (EMR) are a component of the hospital management information system (SIMRS) that is currently being developed. The complexity of the challenges in implementing EMRs necessitates a readiness assessment before they are fully implemented. This is an important step to help identify procedures, levels of importance, and to accommodate the formation of operational functions that support the optimization of EMR implementation. One of the methods that can be used to analyze the readiness for implementing information systems based on EMRs is the Doctor’s Office Quality-Information Technology (DOQ-IT) method. The implementation of EMRs at Mitra Siaga Hospital in Tegal began in April 2024, but it has not yet been fully executed. This study aims to determine the readiness for implementing outpatient EMRs at Mitra Siaga Hospital Tegal using the DOQ-IT approach. This is a descriptive quantitative study with 32 subjects, consisting of IT staff, medical record officers, doctors, and nurses. The results of the study indicate that the readiness for implementing outpatient EMRs at Mitra Siaga Hospital Tegal using the DOQ-IT approach falls into the "very ready" category. Based on the research findings, it is recommended that Mitra Siaga Hospital Tegal continue to monitor, evaluate, and periodically review the implementation of EMRs, as there may still be challenges or obstacles during the implementation process.

Abstract

ABSTRACT Medical records are an essential component of healthcare services as they function to document the entire process of patient care. The development of information technology has encouraged the transformation of medical record management from manual paper-based systems to Electronic Medical Records (EMR), which are considered capable of improving service efficiency, facilitating faster access to patient information, and supporting healthcare financing and reporting systems. However, the implementation of EMR still varies across hospitals and countries due to differences in regulatory frameworks, technological infrastructure readiness, and institutional policies. This study aims to identify and compare the implementation of Electronic Medical Records (EMR) at RSUD Cilacap, Indonesia, and Hospital Sultan Abdul Aziz Shah (HSAAS), Malaysia, in terms of regulation, system integration, casemix system, EMR data processing focus, and Standard Operating Procedures (SOP). This research employed a qualitative descriptive method with a cross-sectional and comparative design. Data were collected through observation and document review as the primary sources, supported by interviews. The results show that RSUD Cilacap implements EMR based on national regulations using a one integrated platform system, integrates EMR with the INA-CBG’s casemix system, and utilizes EMR for both internal and external reporting with SOP management under the medical records unit. In contrast, HSAAS Malaysia implements EMR based on internal hospital policies using multiple platform systems, applies the MY-DRG casemix system which is not integrated with EMR, focuses EMR processing on internal reporting, and manages SOPs under the information technology unit. In conclusion, the implementation of EMR in both hospitals differs in terms of regulation, system integration, casemix system, EMR data processing focus, and Standard Operating Procedures. Keywords: Casemix, Comparative Study, Electronic Medical Record, Integrated System, Regulation.

Abstract

ABSTRACT The Indonesian Case-Based Groups (INA-CBGs) payment system aims to enhance the cost-efficiency of healthcare services; however, implementation has revealed issues with "pending claims." These pending claims can lead to discrepancies between INA-CBG rates and actual hospital costs, particularly in cases with specific severity levels. This study aimed to analyze the relationship between INA-CBG severity levels and the cost discrepancies associated with pending inpatient claims at Dr. H. A. Rotinsulu Lung Hospital, Bandung. A quantitative research design was employed, utilizing a correlational approach with a cross-sectional study design. The study utilized total sampling, comprising 387 medical record files. The variables analyzed were INA-CBG severity levels and claim cost discrepancies. Data analysis was performed using the Chi-Square test. The results indicated that the majority of cases (55.82%) fell under severity level III. Claim discrepancies were predominantly characterized by a deficit, totaling -IDR 642,916,279. The Chi-Square test yielded a p-value of 0.000 (p0.05). The study concludes that there is a significant relationship between INA-CBG severity levels and the cost discrepancies of pending inpatient claims. While severity level is a crucial factor in determining BPJS claim rates, improving the accuracy of medical record documentation and diagnostic coding is essential to minimize pending claims and cost discrepancies, thereby reducing financial losses for the hospital. Keywords: Cost Difference, INA-CBGs, Pending Claims, Severity level. H. A. Rotinsulu Bandung. Abstract

ABSTRACT Tuberculosis remains a public health problem in Indonesia and globally, with uneven distribution of cases across regions. The spread of TB is strongly influenced by environmental and demographic factors, particularly population density. Densely populated areas can increase the risk of bacterial transmission. This study aims to analyze patient characteristics, spatial distribution patterns, and the relationship between population density and the spread of pulmonary TB cases. This study used a correlational design with a cross-sectional approach. The data used were secondary data from 207 pulmonary TB patients at the Dr. H. A. Rotinsulu Lung Hospital in Bandung, and were described univariately, bivariately, and spatially. The results showed that most patients were of productive age (75.3%), male (66.1%), had a high school/vocational high school education (56.06%), and most worked as private employees (22.71%). Patients were spread across various regions. There was a significant relationship (p = 0.032) between population density and the distribution pattern of pulmonary TB, with a positive relationship strength (r = 0.393) in the weak category. The importance of complete and accurate recording and reporting so that it can be used in spatial analysis in identifying regional risks, determining screening priorities, and strengthening regional-based TB control through coordination of health services.  Keywords: Bandung City, Distribution Pattern, Spatial Analysis, Tuberculosis.
