Abstract

Introduction: Dementia significantly impacts elderly quality of life, necessitating innovative non-pharmacological approaches. This study aimed to identify key design elements for multisensory environments integrated with reminiscence therapy in Indonesian contexts. Methods: This qualitative study used an exploratory descriptive design through focus group discussions with 10 elderly participants, 2 family caregivers, and 5 healthcare professionals. Participants were selected through purposive sampling based on inclusion criteria established for this research protocol. Data were analyzed using Braun and Clarke's six-phase thematic analysis framework to identify key patterns related to the development of multisensory environments. Results: This study revealed five main themes from a comprehensive data analysis of therapeutic environments for the Indonesian population. These themes are: 1) the importance of positive reminiscence triggers to effectively stimulate memory, 2) the personal space requirements for comfort and safety as essential environmental design considerations, 3) the need for multisensory integration as a critical component of therapeutic environmental interventions, 4) the vital role of family support and communication strategies in a comprehensive care approach, and 5) the relevance of cultural and spiritual considerations in design to provide contextual meaning. Conclusion: The development of Harmony multisensory spaces should integrate reminiscence therapy principles with personalized design elements, emphasizing safety parameters, comfort metrics, and cultural sensitivity to effectively improve the quality of life for elderly individuals with dementia. Implementation requires multidisciplinary collaboration between healthcare professionals, designers, and family caregivers to ensure optimal therapeutic outcomes.

Abstract

Background: Anxiety is a common psychological problem experienced by patients with Diabetes Mellitus and can negatively affect disease management and quality of life. Mindfulness-Based Stress Reduction (MBSR) combined with 40 Hz music frequency is that may help reduce anxiety and improve quality of life in patients with chronic diseases. Purpose: This study aimed to analyze the effectiveness of MBSR with 40 Hz music frequency on anxiety and quality of life among Prolanis patients with Diabetes Mellitus. Methods: Study design used a quasi-experimental pretest–posttest control group. The sample consisted of 60 respondents, divided into intervention and control groups. The intervention group received an 8-week MBSR program combined with 40 Hz music frequency, while the control group received standard Prolanis care. Anxiety levels using the DASS-21 questionnaire, and quality of life was assessed using WHOQOL-BREF. Data were analyzed using Wilcoxon and Mann–Whitney tests. Results: The anxiety levels significantly decreased in the intervention group (mean difference = −10.15; p < 0.001), while no significant change was observed in the control group (p > 0.05). In addition, quality of life significantly improved in the intervention group (median increase = +16; p < 0.001), whereas the control group showed only a slight and non-significant improvement (p > 0.05). Conclusion: Mindfulness-Based Stress Reduction combined with 40 Hz music frequency is effective in reducing anxiety and improving quality of life among Prolanis patients with Diabetes Mellitus. This intervention can be integrated into primary healthcare services as a complementary nursing intervention to support holistic diabetes management.

Abstract

Background: The managerial capacity of community health center leaders in Indonesia remains limited, while the quality of nursing services depends heavily on how well core management functions are carried out. Purpose: This study aimed to determine the influence of community health center management functions, namely planning, mobilization and implementation, and supervision, on the quality of nursing services, and to identify the most dominant function. Methods: A quantitative observational study with a cross-sectional design was conducted at the Malinau Seberang Community Health Center, North Kalimantan, Indonesia, in 2025. A total sampling of 87 health workers completed Likert-scale questionnaires measuring the three management functions and service quality. Results: Chi-square tests showed significant associations between planning (p=0.007), mobilization and implementation (p=0.007), and supervision (p=0.006) and the quality of nursing services. Multiple logistic regression confirmed that all three functions significantly influenced service quality (p<0.001), with planning showing the largest Exp(B) value (0.0135) and identified as the most dominant determinant. These estimates should be interpreted with caution, however, since the outcome was highly skewed toward the good category and the resulting odds ratios were very small. Conclusion: These findings strengthen the evidence that integrated community health center management is key to improving nursing service quality. Health center leaders should prioritize structured, data-driven planning, supported by coordinated implementation and routine supervision, particularly in remote and border areas.
