Abstract

Low Back Pain (LBP) is one of the most common musculoskeletal disorders experienced by the community, which can decrease functional activity and quality of life. Non-pharmacological management such as the McKenzie Method has been proven effective in reducing pain intensity through repetitive movements and posture correction that help improve spinal mobility, decrease muscle spasms, and restore function.To determine the effectiveness of the McKenzie Method in reducing pain intensity among LBP patients in the Emergency Department of Dr. Tadjuddin Chalid General Hospital Makassar. This study used a case report design on a patient, Mrs. S, who complained of lower back pain radiating to the waist. The nursing intervention involved McKenzie exercises, including prone on elbows, prone press-up, and standing extension, performed under nurse supervision. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Before the McKenzie exercises, the patient's pain scale was 7 (severe pain). After two sessions, the pain scale decreased to 5 (moderate pain), with improved mobility and a more relaxed appearance. The patient’s vital signs remained stable, and no adverse effects were observed during therapy. The McKenzie Method is effective in reducing pain and improving mobility in patients with LBP. This method can serve as a simple, safe, and efficient non-pharmacological nursing intervention for managing lower back pain

Abstract

Hypertension is one of the non-communicable diseases with a high prevalence and is known as the silent killer because it is often asymptomatic and causes serious complications. One of the main problems that hypertensive patients experience is sleep disturbances, which can worsen cardiovascular conditions. Activity Daily Living (ADL) management is an important non-pharmacological strategy to improve sleep quality while helping to control blood pressure. This research was conducted in the Pampang Health Center Working Area with the aim of finding out the effectiveness of the application of ADL management on the sleep quality of hypertensive patients. The research method used a case study on Mrs. T, a hypertensive patient with complaints of sleep disturbances. Interventions were carried out through structured daily activity arrangements including light mobilization, rest time management, low-salt healthy diet, and stress management. The results showed a significant improvement in sleep quality after regular application of ADL for three consecutive days. Before the intervention, the patient was only able to sleep 3–4 hours per night with frequent awakening, while after the intervention the sleep duration increased to 5–6 hours with a better quality and less frequent awakening. The application of ADL also reduces complaints of dizziness during activities and increases patients' independence in carrying out daily activities. In conclusion, ADL management has been shown to be effective as a nonpharmacological intervention to improve the sleep quality of hypertensive patients and can be applied routinely in community nursing care.

Abstract

Hypertension is one of the most prevalent non-communicable diseases and remains a global health problem, often referred to as a silent killer. Elderly individuals with hypertension frequently experience poor sleep quality, which may worsen blood pressure conditions. Objective: This study aims to determine the effect of sleep quality on blood pressure in elderly patients with hypertension. This research employed a case study design on an elderly patient with hypertension at Puskesmas Pampang. The interventions provided included sleep hygiene education, restriction of coffee consumption at night, and regulation of sleep patterns. Evaluation was carried out for three consecutive days by monitoring sleep quality and measuring blood pressure. Results: Before the intervention, the patient had poor sleep quality with blood pressure levels of 167/80 mmHg and 164/85 mmHg. After the intervention, sleep quality improved, complaints of difficulty sleeping decreased, and blood pressure decreased to 162/80 mmHg. Conclusion: Improving sleep quality has an effect on reducing blood pressure in elderly patients with hypertension. Good sleep quality can serve as an important non-pharmacological factor in the management of hypertension

Abstract

Hypertension is one of the non-communicable diseases that is a major cause of premature death worldwide. The prevalence of hypertension in Indonesia reaches 34.11% (Riskesdas, 2018) and in South Sulawesi it is 31.68%. Hypertension can be controlled through pharmacological and non-pharmacological therapies. One non-pharmacological therapy that has been proven effective is Isometric Handgrip Exercise (IHE), which is a static contraction exercise of the hand muscles that can improve endothelial function, reduce sympathetic activity, and lower blood pressure. This study describes the application of Isometric Handgrip Exercise to reduce blood pressure in hypertensive patients in the emergency room of Dr. Tadjuddin Chalid General Hospital in Makassar. The design used was a case report on a 54-year-old male patient with a history of hypertension. Nursing assessment was conducted using the SDKI, SLKI, and SIKI approaches. Interventions included management of increased intracranial pressure, pain management, and Isometric Handgrip Exercise. The patient was admitted with a blood pressure of 208/101 mmHg, a pulse of 107 beats per minute, a headache pain scale of 5, nausea, vomiting, and weakness. After intervention, blood pressure decreased to 180/80 mmHg, pulse rate to 90 beats per minute, and pain scale to 3. The patient appeared more relaxed and able to adapt to the condition. The application of Isometric Handgrip Exercise was proven to help lower blood pressure and reduce pain complaints in hypertensive patients. This exercise is simple, inexpensive, can be done independently, and is safe to be used as a pharmacological adjunct therapy. Nurses are expected to make IHE one of the non-pharmacological interventions in hypertension management, while patients and families are expected to be able to apply this exercise regularly at home to control blood pressure.

Abstract

Pulmonary tuberculosis (TB) remains one of the leading causes of morbidity and mortality in Indonesia. The most common symptoms include shortness of breath, cough, and secretion retention that interferes with airway clearance. One of the non-pharmacological interventions to address respiratory problems is Diaphragmatic Breathing Exercise (DBE), which aims to improve ventilation effectiveness and optimize breathing patterns. Objective: To determine the effectiveness of Diaphragmatic Breathing Exercise in airway management among pulmonary TB patients in the Emergency Department of Dr. Tadjuddin Chalid Hospital, Makassar. Method: A case study was conducted using a nursing care approach, which included assessment, nursing diagnoses, planning, implementation, and evaluation. Results: After the implementation, the patient showed significant improvements, including a more regular respiratory rate (decreasing from 28 breaths/min), reduced use of accessory respiratory muscles, easier sputum expectoration, decreased wheezing, and reduced shortness of breath. These outcomes indicate that DBE is effective in enhancing airway clearance and improving breathing patterns in pulmonary TB patients. Conclusion: Diaphragmatic Breathing Exercise has proven to be an effective non-pharmacological intervention in airway management for pulmonary TB patients. This technique can be used as a supportive therapy in emergency departments as well as in continued care to improve the quality of life of patients.

Abstract

Ureteral stones are one of the most common urological problems that often cause urinary elimination disorders such as retention, severe pain, and risk of complications. Management is generally focused on pharmacological therapy and invasive procedures, while non- pharmacological interventions are still rarely applied. Kegel exercise is a pelvic floor muscle training aimed at improving urinary control and enhancing urinary elimination. To determine the effectiveness of Kegel exercise in improving urinary elimination in the Emergency Department of Dr. Tajuddin Chalid General Hospital, Makassar. This study used a case study design involving a 21-year-old male patient diagnosed with ureteral stones. Nursing care was provided through assessment, diagnosis, intervention, implementation, and evaluation. The main intervention consisted of education and Kegel exercise training.Findings showed improvements in urinary elimination, including smoother urine flow, reduced complaints of incomplete voiding, and decreased pain intensity. Kegel exercise can serve as a simple, safe, and effective non pharmacological intervention to improve urinary elimination. Recommendation: This exercise may be considered as a supportive therapy in emergency nursing care for patients with urinary elimination disorders.
