<![CDATA[Stunting is a condition when toddler's heights below the average ages. Various effort had been done to reduce the number of stunting, still the attempt frequently focus on enhancement child nutrition through the distribution of additional foods. The stunting intervention that compatible with the source of problems needs to be done in order to dissapeare stunting. Within doing the intervention, required the cause of stunting data which consider of many risk factors. Fact on the fieldwork showed data collection of stunting causative factor remains very poor. Based on these problem analysis, we develop a tool to collecting the causative factor of stunting in "Form Dating". This special form developed to help the village healthcare to collect stunting's data. This data would be the stunting intervention plan base that match the source of problem. Form develop through three phase: the trials phase, training phase, and final phase. The action output which conducted in participatory is form DaTing who easily to use by the village healthcares. The form application in big scale can't be done because period limitation of the action.]]>
