Abstrak Anemia merupakan masalah kesehatan penting pada remaja putri di Indonesia dengan prevalensi 32% (Riskesdas 2018). Pemerintah mengimplementasikan program pemberian tablet tambah darah (TTD) di sekolah, namun pemenuhan konsumsi masih rendah, hanya 1,4% sesuai data Dinas Kesehatan Surabaya 2022. Penelitian ini bertujuan menganalisis hubungan antara self-awareness, dukungan guru, dan dukungan teman sebaya dengan kepatuhan konsumsi TTD pada siswi SMPN 58 Surabaya. Metode survei cross-sectional dengan purposive sampling dilakukan pada siswi kelas VII yang menerima TTD minimal tiga bulan. Analisis chi-square dan regresi logistik berganda menunjukkan self-awareness sebagai variabel signifikan (p=0,018, OR=0,364) dan faktor dominan yang mempengaruhi terpenuhinya konsumsi TTD. Remaja dengan self-awareness cukup baik memiliki peluang mencapai 0,364 kali dibandingkan yang baik. Peningkatan kesadaran diri diperlukan melalui kolaborasi antara guru, teman sebaya, orang tua, dan tenaga kesehatan guna mengurangi risiko anemia. The government has implemented a program providing iron supplement tablets (TTD) at schools, but adherence remains low; only 1.4% comply according to Surabaya Health Office data in 2022. This study aims to analyze the relationship between self-awareness, teacher support, and peer support with adherence to TTD consumption among seventh-grade students at SMPN 58 Surabaya. A cross-sectional survey using purposive sampling was conducted on students who had received TTD for at least three months. Chi-square and logistic regression analyses showed self-awareness as a significant variable (p=0.018, OR=0.364) and the dominant factor influencing TTD adherence. Adolescents with moderate self-awareness have 0.364 times the likelihood of adherence compared to those with good self-awareness. Enhancing self-awareness through collaboration among teachers, peers, parents, and health workers is necessary to reduce anemia risk.
