ABSTRAK Entity Relationship Diagram (ERD) adalah diagram yang menggambarkan entitas dalam suatu sistem dan hubungan antar entitas tersebut. ERD terdiri dari beberapa komponen utama, yaitu: Entitas: Merupakan objek data yang menggambarkan informasi penting dalam sistem, seperti objek eksternal, benda, peristiwa, atau peran.Atribut: Menjelaskan karakteristik objek data, seperti nama, warna, atau referensi ke entitas lain.Relasi/Hubungan: Menunjukkan keterkaitan antar entitas, seperti hubungan satu-ke-satu, satu-ke-banyak, atau banyak-ke-banyak. Di Rumah Sakit Umum Daerah Puri Husada Tembilahan, perancangan database yang baik, terutama untuk pengelolaan rekam medis, sangat penting. Rekam medis berisi data pasien yang mencakup biodata lengkap dan riwayat kesehatan. Dengan adanya sistem database berbasis ERD, pengelolaan rekam medis dapat dilakukan lebih efisien, memudahkan operasional rumah sakit, dan meningkatkan mutu pelayanan.
