Pregnancy, childbirth, postpartum, and the newborn period are critical phases requiring continuous care. Continuity of care (COC) midwifery services were provided to Mrs. S, aged 25, from the third trimester of pregnancy until 42 days postpartum, using Varney’s midwifery management approach. Care was delivered at the community health center and the patient’s home through interviews, observations, and physical examinations. The entire process proceeded physiologically without complications. Normal delivery occurred, the baby was born healthy, and the mother remained stable during the postpartum period. The baby received standard care, and the mother chose injectable contraception after counseling. The results indicate that the COC approach is effective in providing comprehensive services and improving the safety of both mother and baby. Suggestions for health workers to maintain the quality of health services and early screening to determine continuous obstetric care and quality of service. Abstrak Kehamilan, persalinan, nifas, dan bayi baru lahir merupakan fase penting yang memerlukan asuhan berkelanjutan. Asuhan kebidanan continuity of care (COC) dilakukan pada Ny. S, 25 tahun, mulai dari kehamilan trimester III hingga 42 hari masa nifas, menggunakan pendekatan manajemen kebidanan Varney. Asuhan dilaksanakan di Puskesmas dan rumah pasien melalui wawancara, observasi, dan pemeriksaan fisik. Seluruh proses berlangsung fisiologis, tanpa komplikasi. Persalinan normal, bayi lahir sehat, dan ibu dalam kondisi stabil selama nifas. Bayi mendapatkan perawatan sesuai standar, dan ibu memilih KB suntik setelah mendapat konseling. Hasil menunjukkan bahwa pendekatan COC efektif dalam memberikan pelayanan komprehensif dan meningkatkan keselamatan ibu dan bayi. Saran bagi tenaga kesehatan untuk mempertahan kualitas pelayanan kesehatan serta skrining awal untuk menentukan asuhan kebidanan yang berkesinambungan dan mutu pelayanan.
