Abstract Diabetes mellitus is a chronic disease whose prevalence continues to rise and which carries a substantial risk of serious complications, largely attributable to the suboptimal self-care capacity of patients. Environmental health education represents a promotive and preventive approach that can be implemented to foster healthy lifestyle behaviors among diabetes mellitus patients. This study aimed to analyze the effectiveness of environmental health education in improving self-care and complication prevention among diabetes mellitus patients at RS Mitra Sejati. This study employed a quasi-experimental design with a one-group pretest–posttest approach. The sample comprised 40 respondents selected through purposive sampling. Data were collected using structured questionnaires assessing self-care and complication-prevention behaviors among diabetes mellitus patients. The intervention was delivered through environment-based health education using lectures, group discussions, leaflets, and demonstrations. The findings revealed an increase in the mean self-care score from 58.42 to 78.65 and in complication-prevention behavior from 55.18 to 80.21 following the intervention. Statistical analysis indicated that environmental health education was effective in improving self-care and complication prevention among diabetes mellitus patients, with a significance value of p<0.05. Environmental health education is therefore recommended as a promotive and preventive strategy in the management of diabetes mellitus.
