Abstract The digital transformation of Indonesia’s healthcare system demands rigorous evaluation of technology acceptance among healthcare professionals, particularly in the use of e-health applications such as e-Puskesmas. Despite increasing adoption, few studies have comprehensively assessed the acceptance of e-Puskesmas using an extended theoretical framework. This study aims to investigate the factors influencing the acceptance and behavioral intention to use the e-Puskesmas application among healthcare workers in Central Lombok. To address this gap, the research employs a modified Technology Acceptance Model (TAM) by integrating four external variables Information Quality, System Quality, Service Quality , and Computer Self-Efficacy as well as Digital Literacy as a moderating variable. A quantitative explanatory approach was used, involving 505 respondents selected through simple random sampling. Data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) with the SmartPLS software. The findings reveal that information quality, system quality, service quality, and computer self-efficacy significantly affect both perceived ease of use and perceived usefulness. These, in turn, influence user satisfaction, attitude toward using, and behavioral intention to use the system. Furthermore, digital literacy significantly moderates the relationship between perceived usefulness and ease of use with satisfaction and attitude. This research contributes theoretically by extending the TAM framework in the e-health domain and provides practical insights for policymakers to enhance system quality, service delivery, and digital skills training for healthcare workers to improve digital health adoption at the regional level.
