Wire hoisting crane is an essential component in ship cargo handling operations, functioning as a load lifting device. Damage to the wire hoisting crane, particularly wire failure, can disrupt cargo operations and increase the risk of occupational accidents. This study aims to analyze the causes of wire hoisting crane failure during cargo handling operations on MV AGP KARTINI III and to examine the role of routine maintenance and inspection in its prevention. The research method employed is descriptive qualitative, with data collected through field observations, interviews with ship crews, and documentation studies. The results indicate that wire hoisting crane failure is caused by physical deterioration of the wire due to wear and corrosion, crane operation with loads approaching or exceeding the Safety Working Load (SWL), and inadequately implemented maintenance and inspection practices. Therefore, planned maintenance, regular inspections, and proper crane operation in accordance with safety procedures are necessary to prevent similar incidents.
