<![CDATA[Patient identification is part of patient safety standards that must be implemented by healthcare workers in providing care in hospitals. Nurses and midwives are healthcare professionals who observe and deliver services 24 hours a day. Knowledge, attitude, and motivation influence patient identification practices. This study aims to describe knowledge, attitudes, motivation, and patient identification practices in the inpatient wards of RSPI Sulianti Saroso during the period of October–November 2024. This was a descriptive study with a cross-sectional design involving 150 nurses, midwives, and head nurses using purposive sampling. The instrument used was a valid and reliable research questionnaire X. Descriptive statistical analysis was presented in frequencies (n) and proportions (%). Most nurses and midwives had good knowledge, attitudes, and motivation, and head nurses stated that all nurses/midwives performed well in patient identification. However, many still showed misconceptions in shift-to-shift communication, hand hygiene, fall risk assessment, and family education. The study recommends that hospitals improve patient safety through reporting technology and educational media.]]>
