Bow leakage is a structural failure that can threaten vessel safety and disrupt maritime operations. This study aims to analyze the occurrence of bow leakage on KMP Safira Nusantara following a collision during a berthing maneuver. The research employed a qualitative descriptive method using a case study approach. Data were collected through direct observation, interviews with ship officers and crew members, documentation, and analysis of incident reports related to the accident. The findings revealed that the leakage did not occur suddenly but resulted from a series of events during the berthing process. A malfunction in the main starting valve caused the propulsion system to respond inadequately, limiting the vessel’s ability to correct its position while approaching the berth. As a consequence, the bow section collided with the pier, generating impact forces that caused deformation of the bow plating and cracks in the welded joints below the waterline. These structural damages created openings that allowed seawater to enter the forepeak compartment, resulting in leakage. The study concludes that technical failures in critical propulsion components during maneuvering operations can significantly affect ship controllability and structural integrity. Therefore, proper maintenance of propulsion systems and strict adherence to maneuvering procedures are essential to minimize the risk of collisions and prevent structural damage that may lead to leakage.
