The Influence of Digital Health Literacy and Family Support on Medication Adherence through Health Belief

Abstract

Disciplined medication adherence is a determining factor for therapeutic success and the prevention of fatal complications in hemodialysis patients. However, lifelong medication routines often trigger treatment fatigue, thereby reducing adherence levels. This study aims to analyze the effect of digital health literacy and family support on medication adherence, as well as to examine the role of the Health Belief Model (HBM) as a moderating variable. This quantitative study, employing a cross-sectional design, involved 72 hemodialysis patients at Hospital X, selected through a total sampling approach. All variables were measured using a 1–4 scale questionnaire instrument and analyzed using Structural Equation Modeling with Partial Least Squares (SEM-PLS), assisted by SmartPLS software. The final model demonstrated a good data fit (SRMR = 0.094) and simultaneously explained 72.7% of the variance in medication adherence (R² = 0.727). The results showed that digital health literacy, family support, and HBM partially had a positive and significant effect on medication adherence. Among all predictors, family support was the strongest determinant, operating dominantly and independently of the patients' beliefs. Meanwhile, HBM significantly moderated and strengthened the effect of digital health literacy on medication adherence. In conclusion, digital literacy acts as a catalyst that is only effective in improving adherence when built upon a strong foundation of the patients' health beliefs (HBM).
