<![CDATA[Background: Hypertension morbidity remains high due to low treatment adherence triggered by the perception of "feeling healthy." The use of digital technology for self-care is not yet optimal. This study aims to develop a "Totok Punggung" (Back Acupressure) telenursing application based on Orem's Theory and test its effectiveness on adherence and blood pressure. Subjects and Method: The research design used mixed-methods (Research and Development followed by a Randomized Controlled Trial). The study was conducted in Malang Regency with a sample of 100 hypertensive patients (50 intervention, 50 control) selected through multistage random sampling. Research instruments included Orem's self-care ability questionnaire, CULIG adherence scale, and digital sphygmomanometer. Data analysis used Paired and Independent T-Tests. Results: The application was declared valid (CVI=0.92) and had high acceptability (SUS score=78.5). The intervention group showed a significant increase in self-care ability (p<0.001) and adherence (p<0.001). There was an average decrease in systolic blood pressure of 24.20 mmHg and diastolic of 12.20 mmHg in the intervention group (p<0.001) compared to the control group. Conclusion: The "Totok Punggung" telenursing application is effective in bridging self-care deficits, changing non-compliant behavior, and stabilizing patient hemodynamics.
