Scabies infestation by Sarcoptes scabiei cause crust formation, skin damage, and impaired wound healing. This study evaluated the effectiveness of combined sulfur and aloe vera therapy on crust length and collagen density in scabies-infected white rats. A laboratory experimental study with a completely randomized design was conducted. Twenty-five male Wistar rats were divided into five groups: negative control (healthy, no infection), positive control (infected, no therapy), and three treatment groups (P1, P2, P3) infected and treated with a gel containing sulfur (5%, 10%, and 15%, respectively) in aloe vera gel for 7 days. Crust length was measured using digital calipers, and collagen density was assessed semi-quantitatively (score 0–4) on Hematoxylin-Eosin-stained ear skin sections. Data were analyzed using One-way ANOVA followed by Tukey’s HSD (crust length) and Kruskal-Wallis followed by Mann-Whitney U test (collagen density). Combined therapy significantly reduced crust length (p=0.006) and increased collagen density (p<0.05). The 5% sulfur group (P1) showed the most significant crust reduction (p=0.005) and collagen density improvement (p=0.003) compared to the positive control. In conclusion, the combination gel of aloe vera and 5% sulfur effectively reduces crust length and improves collagen density, accelerating wound healing in scabies-infected rats.
