Abstract

Background: Chronic Kidney Disease (CKD) is characterized by a progressive decline in kidney function, which can cause various complications and increase the potential for drug-related problems (DRPs). DRPs are events that link actual or potential drug therapy to patient health outcomes. Objective: This study aimed to determine the effect of DRPs on the outcomes of CKD patient therapy. Methods: This study used a retrospective approach with sampling conducted at Muhammadiyah Hospital Lamongan from January 2024 to December 2024. The study population consisted of 250 patients, and the research sample was selected using purposive sampling, which met the inclusion criteria, resulting in a total sample of 155 patients. The inclusion criteria were inpatients suffering from CKD with or without complications, CKD patients with stage ≥ 2, and an age range of ≥ 25 years. Results: The findings of this study indicate that DRPs in CKD patients were due to drug interactions (78,07%), drug duplication (10,32%), and polypharmacy (24,51%). The outcome of therapy in CKD patients was based on achieved GFR values (76,78%) and those not achieved (23,22%). The results of the study showed that there was influenced the outcome of therapy, with  a p-value of 0,040. Conclusion: The influence of DRPs on therapeutic outcomes in patients has is significant, with their presence reducing the effectiveness of therapy and increasing the risk of complications. Therefore, early identification and management of DRPs are necessary to achieve optimal therapeutic results.
