Visual impairment due to refractive error in adolescents can hinder learning concentration and productivity, particularly at the senior high school level, which demands intensive reading and high digital device use. The Community Service Program "Mata Sehat Anak Cerdas" (Healthy Eyes, Smart Children) was carried out at SMA Negeri 30 Jakarta to conduct visual acuity screening, early detection of refractive errors, eye health education, and eyeglasses provision for grade 12 students and teachers. The method included anamnesis, pupillary distance measurement, lensometry, visual acuity screening using a Snellen chart, objective and subjective refraction examination, and eyeglasses distribution for those in need. The target consisted of 220 respondents, comprising grade 12 students (classes 12-1 to 12-8) as well as teachers and staff. Results showed that 97 respondents (44.1%) were identified with refractive errors and received eyeglasses, while 123 respondents (55.9%) were emmetropic. The highest prevalence was found among teachers and staff (89%), followed by class 12-1 (54%). This activity underscores the importance of school-based eye health screening at the senior high school level as a sustainable preventive and promotive effort.
