Hypertension and diabetes mellitus are chronic non-communicable diseases that pose a risk of complications without clear symptoms. This community service program aimed to enhance community knowledge and awareness regarding risk factors, prevention, management, medication adherence, and the importance of early detection. The target audience was the community of Sangkal Hamlet, Bangunharjo Village, Sewon District, Bantul Regency, with a total of 25 participants consisting of adults and the elderly. The program was conducted on July 16, 2026. The methods included health education lectures, interactive discussions, blood pressure and random blood glucose (RBG) screenings, individual counseling, and knowledge evaluation using a one-group pre-test and post-test design. The results demonstrated an increase in the mean knowledge score by 18.5 points, rising from 65.5 (pre-test) to 84.0 (post-test). A total of 20 participants (80%) showed an increase in scores, while 5 participants (20%) maintained constant scores. Health screening results revealed that 18 participants (72%) had blood pressure levels ?130/85 mmHg and 2 participants (8%) had RBG levels ?200 mg/dL. The combination of interactive education and health screening effectively improves community knowledge and awareness toward the prevention of non-communicable diseases. Nevertheless, further evaluations are required to assess the sustainability of long-term behavioral changes.
