Telecommunication towers are vital infrastructures that support modern communication systems. Structural failure of telecommunication towers may lead to technical, economic, and public safety impacts. This study aims to analyze the causes of the collapse of a 72-meter Self Supporting Tower (SST) located in Ohoirenan Village, Southeast Maluku Regency. The research employed a case study method with a descriptive approach and technical analysis based on the ANSI/TIA-222-G standard. The analysis results indicate that the structural failure was caused by the loss of several structural elements, including bracing members, connection plates, and bolts, as well as bolt tightness that did not meet the required standards. In addition, extreme wind loads and inadequate maintenance contributed to accelerating the structural collapse. From a non-technical perspective, violations of ethics, weak professionalism, and suboptimal implementation of Occupational Health, Safety, and Environment (OHSE) practices were also identified. This study recommends improving design quality, construction supervision, and the implementation of stricter safety management systems to prevent similar incidents in the future.
