Noncommunicable diseases (NCDs) require continuous early detection and risk-factor monitoring at the community level. Posbindu PTM plays a strategic role in NCD prevention and control; however, its implementation continues to face challenges related to data recording, health-history monitoring, reporting, and data integration. This study aimed to develop the Smart Village Sehat Model as an approach to optimize the role of Posbindu PTM through public health information technology. The study employed a Research and Development (R&D) method with a mixed-methods approach and was conducted in Bluru Kidul Village, Sidoarjo Regency. The research stages included needs assessment, initial model development, expert and practitioner validation, model revision, limited testing, and final model development. The needs assessment indicated high demand for a digital health information system (90.0%), continuous risk-factor monitoring (86.7%), and improvement of manual recording practices (80.0%). The overall model validation score reached 77.7%, indicating that the model was feasible. Limited testing achieved an average score of 87.1%, with the highest scores for benefits to health cadres (92%) and ease of data recording (90%). The resulting model integrates communities, Posbindu PTM, health information technology, primary health centers, and village governments to support continuous health monitoring and data-driven decision-making at the village level.
