Infertility is a reproductive health problem that can affect the well-being of individuals and couples of reproductive age. Advances in technology have encouraged the use of online-based pregnancy program consultations as an alternative reproductive health service. Clients’ perceptions of the service and their adherence to recommendations are important aspects of successful consultations. This study aimed to describe clients’ perceptions of adherence levels in online-based pregnancy program consultations at the Euis Urujiyah Midwifery Independent Practice, S.Tr.Keb.Bdn, in 2026. This study used a descriptive method with a qualitative approach. Six informants participated, consisting of four clients and two service providers. Data were collected through in-depth interviews and document review, then analyzed using the Miles and Huberman interactive model. Data validity was ensured through source and method triangulation. Clients’ perceptions of online-based pregnancy program consultations included ease of access, communication and information, responsiveness, and perceived benefits of the service. Online consultations were considered to facilitate access to information and services. Client adherence was reflected in following recommendations and monitoring during the pregnancy program with support from service providers. Online-based pregnancy program consultations facilitate access to services and information and support clients’ adherence to recommendations during the pregnancy program. Service providers are expected to improve the quality of communication, information delivery, client monitoring, and follow-up in online-based pregnancy program consultations.
