Chronic Energy Deficiency (CED) during pregnancy is a prolonged state of energy and protein insufficiency that increases health risks for both mother and fetus. Monitoring nutritional status alongside continuous antenatal care is essential for comprehensive maternal assessment. This case report aims to describe the assessment, analysis, management, and clinical progress of a third-trimester primigravida with CED using the SOAP approach. The subject was Mrs. L, a 28-year-old G1P0A0 woman at 26–29 weeks of gestation with an initial Mid-Upper Arm Circumference (MUAC) of 21.5 cm, treated at Independent Midwifery Practice (PMB) "S" in Klampis. Data were collected through anamnesis, physical and obstetric examinations, laboratory tests, documentation review, and literature study. Care was provided over three visits on June 25, June 30, and July 3, 2026. Interventions included education on CED and balanced nutrition, supplementary feeding, UHT milk, Fe, Multiple Micronutrient Supplements (MMS), calcium, psychological support, nutritional monitoring, and non-pharmacological management of tingling. Body weight increased from 42 kg to 45.5 kg, while MUAC increased to 22 cm. By the third visit, tingling had subsided, Hb was 11.4 g/dL, urine albumin was negative, and maternal-fetal conditions remained stable. However, as MUAC remained below 23.5 cm, CED was not yet resolved. Continued nutritional monitoring, supplementation, fetal growth assessment, and ANC are recommended until delivery.
