Case Report pada Primigravida Trimester III dengan Preeklamsia di PMB ā€œIā€ Klampis Kabupaten Bangkalan

Abstract

Preeclampsia is a hypertensive disorder of pregnancy that requires continuous maternal and fetal surveillance. In a third-trimester primigravida, the clinical condition may coexist with anxiety related to childbirth, requiring midwifery care that addresses both clinical and psychological needs. This case report aimed to describe SOAP-based midwifery care for a third-trimester primigravida with preeclampsia at PMB ā€œIā€, Klampis, Bangkalan Regency. A descriptive single case report was conducted involving Mrs. S, 21 years old, G1P0A0. The initial assessment at 34-35 weeks of gestation showed blood pressure of 140/90 mmHg and a history of urine protein +3. Data were obtained through history taking, vital-sign and obstetric examinations, Mean Arterial Pressure (MAP) and Roll Over Test (ROT), urine protein examination, ultrasonography results, and care documentation. Follow-up was conducted during four visits on June 22, June 30, July 4, and July 6, 2026. Care included education regarding warning signs, rest and nutrition, psychological support, adherence to obstetric review and prescribed therapy, and prenatal yoga and self-hypnosis as complementary interventions. During follow-up, blood pressure ranged from 130-140/90 mmHg, MAP from 103-106 mmHg, and ROT remained 15 mmHg. Urine protein, previously +3, was recorded as negative on July 4 and July 6, 2026. Fetal heart rate remained 135-145 beats/minute with cephalic presentation, while maternal anxiety gradually decreased. SOAP-based care supported systematic longitudinal monitoring, but the observed changes cannot be attributed solely to the complementary interventions because medical follow-up, prescribed treatment, health education, and family support continued throughout the case.
