Abstract

Background: Calcaneal spur is a musculoskeletal condition that may cause heel pain, limited ankle range of motion, decreased muscle strength, and impaired functional activity, particularly during standing, walking, and weight-bearing activities. These impairments may limit daily activities and require physiotherapy interventions to reduce symptoms and improve functional movement. Objective: This study aimed to describe changes in pain, ankle joint range of motion, muscle strength, and functional activity following the application of Ultrasound Therapy and Manual Therapy in a patient with an inferior and posterior calcaneal spur of the left calcaneus. Methods: A case study design was applied to a male patient diagnosed with an inferior and posterior calcaneal spur of the left calcaneus. The intervention was administered over four sessions and consisted of Ultrasound Therapy and Manual Therapy in the form of deep friction massage, joint mobilization, and stretching. Outcomes were evaluated using the Visual Analogue Scale (VAS), goniometer, Manual Muscle Testing (MMT), and Foot and Ankle Disability Index (FADI). Results: Pressure pain decreased from 5.2 to 4.2, while movement pain decreased from 3.4 to 2.8. Active ankle dorsiflexion increased from 15° to 20°, whereas plantarflexion remained at 50°. Muscle strength for dorsiflexion and plantarflexion increased from grade 3 to grade 4. The FADI score increased from 65 (73.9%) to 78 (88.6%), indicating improved functional activity from mild impairment toward near-normal status. Conclusion: Four sessions of Ultrasound Therapy and Manual Therapy were associated with improvements in pain, ankle dorsiflexion range of motion, muscle strength, and functional activity in a patient with a left-sided calcaneal spur.
