Background: Discharge planning is an essential component of nursing care aimed at preparing patients and their families to continue care appropriately after hospital discharge. High-quality discharge planning helps patients understand their health conditions, follow-up care, medication use, scheduled follow-up appointments, and signs and symptoms that require attention. Purpuse: To describe patients’ perceptions of the quality of discharge planning implementation among inpatients at Meuraxa Regional General Hospital, Banda Aceh, in 2026. Method: This study employed a quantitative descriptive design. The study population consisted of patients receiving care in three inpatient wards. A total of 93 respondents were selected using simple random sampling. Data were collected using a respondent characteristics form and a 15-item discharge planning quality questionnaire, which had been declared valid and reliable, with a Cronbach’s α of 0.89. Data were analyzed descriptively and presented as frequency distributions and percentages. Results: The results showed that most respondents perceived the quality of discharge planning as good, with 83 respondents (89.2%) reporting a good perception. Meanwhile, 9 respondents (9.7%) perceived the quality as fair, and 1 respondent (1.1%) perceived it as poor. Conclusion: Most patients perceived the quality of discharge planning as good. The hospital is expected to maintain and improve the quality of education, communication, and patients’ and families’ understanding of home care.
