<![CDATA[Health cadres play a critical role in supporting primary healthcare delivery at the community level; however, their effectiveness is often constrained by fragmented management systems and limited integration with formal health services. This study aimed to develop a comprehensive community nursing model for managing health cadres in primary care settings. A qualitative case study design was employed involving 25 participants, including health cadres, nurse supervisors, and primary healthcare managers. Data were collected through in-depth interviews, focus group discussions, and document reviews, and analyzed using thematic analysis. The findings identified four key domains forming the proposed model: strategic planning, capacity development, supervision and performance evaluation, and community engagement, supported by enabling factors such as leadership, communication, resources, policy support, and information systems. The results revealed that the absence of structured management processes, limited continuous training, and inconsistent supervision negatively affected cadre performance. Conversely, integrated and community-oriented management practices enhanced coordination, competency, and service delivery effectiveness. The proposed community nursing model provides a systematic and interconnected approach to strengthening health cadre management and improving primary healthcare services.]]>
