Adolescents are a vulnerable age group to various physical, mental, and behavioral health problems due to limited understanding of healthy lifestyles, self-esteem, and wise decision-making. Similar conditions were identified among adolescents in Buyat Selatan Village. This community service activity aimed to improve adolescents’ knowledge, attitudes, and behaviors through the Buyat Selatan SEHATI Program (Healthy, Self-Esteem, Wise Actions for the Future). The program employed an educative-participatory approach, including health education sessions, group discussions, simulations, and adolescent mentoring over a two-week period. Evaluation was conducted using pre-test and post-test assessments and behavioral observations. The results demonstrated a significant improvement in adolescents’ knowledge regarding healthy behaviors, mental health, and self-esteem. The average knowledge score increased from a moderate to a good category, accompanied by increased active participation in educational activities. Furthermore, adolescents exhibited positive attitudinal changes toward maintaining health, valuing themselves, and making wiser life decisions. The SEHATI program proved to be an effective community-based intervention model for strengthening adolescent capacity. In conclusion, the Buyat Selatan SEHATI Program successfully enhanced adolescents’ health awareness and behaviors while strengthening self-esteem as preparation for a better future. This program is recommended for sustainable implementation with cross-sectoral support.
