Worm infections remain a common health issue among elementary school-aged children in Indonesia, especially in environments where the implementation of Clean and Healthy Living Behaviors (PHBS) is not optimal. This community service activity aims to improve knowledge and PHBS behaviors as an effort to prevent worm infections among students at SD Negeri 015 Samarinda Ulu. The activity was carried out using a participatory-educative approach through several stages: coordination with the school, interactive counseling using educational media, hand-washing demonstrations following WHO guidelines, and evaluation through pre-test and post-test. The results showed a significant increase in the students' knowledge levels. Before the activity, most students were in the medium and low knowledge categories, which then significantly increased to 92.8% in the high category after health education was provided. In addition to the knowledge improvement, this activity also encouraged positive behavioral changes, such as the habit of washing hands before meals and after activities, as well as regularly maintaining nail hygiene. Overall, this activity proved effective in raising awareness, shaping attitudes, and strengthening healthy living behaviors from an early age in elementary school children as a preventive measure against ongoing worm infections in the community.
