<![CDATA[Sensory perception disturbances, particularly hallucinations, are common psychiatric issues encountered in mental health nursing services. Hallucinations are symptoms of mental disorders in which an individual experiences altered sensory perceptions of internal or external stimuli, accompanied by diminished, excessive, or distorted responses. This study used a case study method involving one patient with auditory hallucinations treated at Grhasia Mental Hospital, Wisma Sembodro. A holistic nursing approach was applied through comprehensive assessment, diagnosis formulation based on the Indonesian Nursing Diagnosis Standard (SDKI), and intervention planning aligned with the Indonesian Nursing Outcomes Classification (SLKI) and Nursing Interventions Classification (SIKI). Nursing interventions included techniques such as confronting hallucinations, distraction through conversation, scheduled activities, and enhancing medication adherence. The results showed a noticeable change before and after the interventions in the patient with sensory perception disturbance: auditory hallucinations. Evaluation indicated gradual improvement in the patient’s ability to control and redirect hallucinatory stimuli, as reflected by increased scores in pre- and post- intervention assessments, although the symptoms did not fully disappear. It is recommended that future research involve more patients to obtain more diverse and comparable results. The therapy duration should also be extended to better observe patient progress over time.]]>
