Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a group of progressive pulmonary disorders characterized by airflow limitation in the respiratory tract. Globally, COPD represents a major health burden, accounting for more than 3 million deaths annually, and is projected to become the third leading cause of death worldwide by 2020. This study aims to implement nursing care for a palliative COPD patient experiencing spiritual distress in the Dahlia Ward of Wates Regional Hospital. This research utilized an observational case study design with a cross-sectional approach. The subject of this case study was a 53-year-old male patient, Mr. S, hospitalized at Wates Regional Hospital. The patient's palliative care screening score was 6, indicating the need for referral to palliative services. The Edmonton Symptom Assessment System (ESAS) revealed that the most prominent issue was spiritual distress, rated at a level of 7. The patient's Karnofsky Performance Status score was 50%, indicating frequent need for assistance and regular medical care. The Spiritual Well-Being Scale (SWBS) yielded a total score of 41, suggesting a moderate level of spiritual well-being. Spiritual assessment indicated that the patient refrained from performing prayers during illness, believing that illness justified this decision. Based on the assessment, Mr. S was identified as experiencing spiritual distress associated with his medical diagnosis of COPD. The primary nursing diagnosis was spiritual distress related to lifestyle changes, based on the Indonesian Nursing Diagnosis Standard (SDKI). The expected nursing outcomes, according to the Indonesian Nursing Outcomes Standard (SLKI), were improvements in spiritual status, particularly the ability to engage in worship and enhanced coping abilities. Interventions were provided based on the Indonesian Nursing Intervention Standard (SIKI), focusing on supporting the patient's ability to perform religious practices.

Abstract

Introduction: Nurses bear significant responsibility in caring for patients, often leading to high workloads, stress, or burnout. One factor that can help reduce burnout is social support from coworkers. This support serves as a source of assistance within the workplace environment, enabling nurses with high levels of social support to better manage burnout. This study aims to investigate the relationship between coworker social support and burnout among nurses working in the inpatient wards of PKU Muhammadiyah Gamping Hospital. Methods: This study employed correlational research design with cross-sectional approach. The participants included 60 nurses working in the inpatient wards, selected using non-probability sampling technique. Results: The Spearman-Rank test showed a significant relationship between coworker social support and burnout among inpatient ward nurses (p-value 0.000 < 0.05). The strength of the relationship was very high, with a correlation coefficient of τ = -0.840 (very strong negative correlation). Conclusion: There is a significant relationship between coworker social support and burnout among inpatient ward nurses at PKU Muhammadiyah Gamping Hospital. This study provides valuable insights for fostering coworker social support as a preventive measure against burnout among inpatient ward nurses.

Abstract

Postoperative mobilization following a Cesarean section presents a significant challenge in the recovery process, particularly within the Enhanced Recovery After Cesarean Surgery (ERACS) protocol, which emphasizes the importance of early movement within the first six hours. One of the primary barriers to mobilization is postoperative pain, a factor that has not been extensively studied specifically in the context of ERACS. This study aims to examine the relationship between pain intensity and the level of mobilization in post-Cesarean section patients managed with the ERACS method. A descriptive correlational design with a cross-sectional approach was employed. A total of 51 patients were selected using a total sampling technique at PKU Muhammadiyah Bantul Hospital. Research instruments included the Numeric Rating Scale (NRS) for pain assessment and an observation sheet for mobilization recorded at 2, 4, and 6 hours postoperatively. The results showed that the majority of patients experienced mild pain (62.7%) and achieved mobilization within the time specified by the protocol (66.7%). Chi-square analysis revealed a significant relationship between pain level and mobilization (p = 0.000), with a moderate correlation strength (contingency coefficient = 0.498). These findings highlight the critical role of effective pain management in supporting timely mobilization according to the ERACS protocol and in accelerating the overall postoperative recovery process.

Abstract

ABSTRACT (Due to the critical condition of patients and uncertainty of prognosis, families of patients admitted to the ICU often experience high anxiety. Coping mechanisms are very important to handle the anxiety of the patient's family. Families can use effective coping mechanisms to develop their own adaptive coping mechanisms.) Introduction: The purpose of this study is to see how family coping mechanisms and patient anxiety in the ICU room of PKU Muhammadiyah Gamping Hospital relate to each other. Methods: This study used a quantitative design with a crosssectional approach, and the total sampling method was used. The study sample consisted of 30 family members of patients admitted to the ICU room. The tools used were the brief cope inventory (BCI) questionnaire, which was used to measure coping mechanisms, and the Hamilton anxiety rating scale (HARS) questionnaire, which was used to measure anxiety levels. Bivariate analysis and chi-square statistical test were used to analyze the data. Results: The results showed that most respondents used adaptive coping mechanisms (63.3%), while 36.7% used maladaptive coping mechanisms. Correlation analysis showed a significant relationship between coping mechanisms and family anxiety (P<0.05), where more adaptive coping mechanisms were associated with lower anxiety levels. Conclusion: In conclusion, there is a significant relationship between coping mechanisms and anxiety of the patient's family in the ICU. Nursing interventions that support the strengthening of adaptive coping mechanisms can help reduce the anxiety of the patient's family during treatment in the ICU.
