Bowel preparation quality determines colonoscopy success; non-adherence and anxiety may reduce patient readiness. This study examined the effect of a pre-colonoscopy educational video on bowel preparation adherence and anxiety levels. A quasi-experimental nonequivalent control group pretest-posttest design was used. Fifty-six respondents recruited through consecutive sampling were divided into an intervention group (n=28) receiving additional educational video and a control group (n=28) receiving standard information. Adherence was measured using a questionnaire, anxiety using HADS-A, and bowel quality using BBPS. Data were analyzed using Wilcoxon, Mann-Whitney, and multiple linear regression. The intervention group showed a decrease in median anxiety from 13.00 to 8.00 (p<0.001), while the control group decreased from 12.00 to 10.50 (p<0.001). The intervention group had higher adherence (median 18.00 vs 15.00; p<0.001). Adherence was the dominant factor influencing BBPS (B=0.402; p<0.001). The educational video improved adherence and reduced anxiety. These findings support integrating educational videos as a standardized nursing intervention for pre-colonoscopy preparation to improve bowel preparation quality and patient psychological comfort.
