The Center for Forecasting Plant Pest Organisms (Balai Besar Peramalan Organisme Pengganggu Tumbuhan/BBPOPT) plays a strategic role in managing laboratory testing services to support national crop protection. However, the previous operational service mechanism operated semi-manually, causing bottlenecks in tracking application statuses, assigning laboratory analysts, verifying payment proofs, and conducting multi-stage validation for test reports and certificates. This study aims to design and develop a web-based Laboratory Testing Service Information System that integrates a multi-stage approval workflow and Role-Based Access Control (RBAC) across seven user actors using the Laravel 12 framework. Software development followed the Prototype methodology comprising five systematic phases: communication, planning, modeling, construction, and deployment. Functional evaluation was conducted via Black-Box Testing on 16 test scenarios (8 front-end and 8 back-end scenarios), covering application submission, payment verification, role-based route authorization security, and document validation. The Black-Box test results demonstrated 100% functional compliance (valid). Furthermore, User Acceptance Testing (UAT) involving 20 participants (10 service users and 10 internal staff across six back-end roles) achieved a 100% acceptance rate. Consequently, the developed system is functionally validated and well accepted by end users in supporting laboratory testing service governance at BBPOPT.
