<![CDATA[The digital transformation of healthcare services has encouraged healthcare facilities to implement Electronic Medical Records (EMRs) to improve service quality, administrative efficiency, and health information management. The Koja District Community Health Center, North Jakarta, has implemented an EMR system through the ENA PRIMER application since 2017; however, several challenges continue to hinder its optimal utilization. This study aimed to analyze the implementation of EMRs and identify the barriers affecting their implementation based on the 5M approach (Man, Money, Material, Machine, and Method). A qualitative descriptive design was employed, with data collected through observations and in-depth interviews involving medical record officers, registration staff, and information technology personnel. The findings revealed that EMR implementation has been supported by Standard Operating Procedures (SOPs) and adequate technological infrastructure. Nevertheless, the implementation has not yet achieved optimal performance due to network disruptions, limited application features, dependence on external vendors for system development, and the incomplete adoption of EMRs across all service units. Based on the 5M analysis, barriers were identified in staff competency for handling system-related issues, lengthy budget approval processes, limitations in application development, network infrastructure problems, and implementation methods that have not been fully integrated. Strengthening human resource capacity, improving information technology infrastructure, and developing a more adaptive system are necessary to optimize EMR implementation at the Koja District Community Health Center.]]>
