Workplace accidents pose major challenges in the shipyard industry due to high-risk activities like welding, blasting, and heavy lifting. This descriptive observational study aimed to identify, evaluate, and control safety risks at PT. Dok da Perkapalan Kodja Bahari (DKB) 1 Jakarta, specifically within the Blasting & Painting and Hull Replating sections. Utilizing purposive sampling, data were collected via field observations, safety records, and worker interviews. Data analysis integrated Failure Mode and Effect Analysis (FMEA) to determine Risk Priority Numbers (RPN) alongside Job Safety Analysis (JSA). The process identified 24 potential risks in Blasting & Painting and 21 in Hull Replating, with critical hazards including high-pressure exposure and electrical shocks. Based on RPN calculations, risks were categorized into high, medium, and low levels. Control strategies were formulated using the hierarchy of hazard control, incorporating engineering, administrative protocols, and mandatory PPE. In conclusion, integrating FMEA and JSA successfully categorized operational hazards and established targeted control measures, enhancing the company’s safety management system.
