Abstract

Beta-thalassemia is a hereditary blood disorder that leaves patients dependent on regular blood transfusion, placing them at risk of iron overload, which can be monitored through ferritin levels and controlled through chelation therapy, though success depends on adherence. Studies examining transfusion frequency and chelation adherence together in relation to ferritin remain limited, so this study aimed to analyze the relationship between ferritin levels and both transfusion frequency and chelation adherence in beta-thalassemia patients. An analytic observational design with a cross-sectional approach was used, involving 36 patients selected through purposive sampling. Ferritin and transfusion frequency data were obtained from medical records, while adherence was measured by questionnaire, then analyzed through univariate, bivariate, and multivariate analysis with multiple linear regression. Results showed a high mean ferritin level, with significant differences between high and low adherence groups, and between patients transfused once versus twice monthly. In the multivariate analysis, chelation adherence remained significantly associated with ferritin levels, whereas transfusion frequency lost significance once both factors were analyzed together, with no significant interaction found. These findings indicate that chelation adherence plays a more dominant role than transfusion frequency, suggesting that iron overload management should combine ferritin monitoring, transfusion adjustment, and stronger adherence support to reduce long-term complications.
