Type 2 Diabetes Mellitus (T2DM) is a chronic non-communicable disease with a steadily rising global and local prevalence, in which unhealthy lifestyle choices play a critical role. This study aimed to analyze the association of lifestyle factors (dietary pattern, physical activity, sleep pattern, stress, and smoking habits) with the incidence of T2DM at Stabat Public Health Center. A quantitative study with a cross-sectional design was conducted on 1,123 T2DM patients recorded in 2025, with a sample of 92 respondents selected using Slovin's formula. Data were collected through questionnaires and analyzed using univariate, bivariate (Chi-Square), and multivariate (multiple logistic regression) analyses. Bivariate analysis revealed significant associations between T2DM incidence and dietary pattern (p = 0.002; OR = 8.333), physical activity (p = 0.000; OR = 13.839), sleep pattern (p = 0.008; OR = 4.059), stress (p = 0.000; OR = 11.420), and smoking habits (p = 0.021; OR = 3.235). Multivariate analysis confirmed that physical activity was the most dominant lifestyle factor associated with T2DM incidence (p = 0.008; OR = 10.091; 95% CI: 1.820–55.947). All evaluated lifestyle factors are significantly associated with T2DM, with physical inactivity identified as the primary determinant. Stabat Public Health Center is recommended to strengthen promotive-preventive programs focusing primarily on physical activity and comprehensive healthy lifestyle interventions.
