The increasing coverage of participants in Indonesia’s National Health Insurance requires the provision of healthcare services that are optimal, efficient, and sustainable. The Referral Back Program was developed to support the management of stable chronic disease patients through services provided at primary healthcare facilities and Referral Back Program pharmacies. Referral Back Program pharmacies play a crucial role in ensuring medication access and continuity of therapy, but their implementation still faces various challenges that could affect service quality. By reviewing various scientific articles, regulations, and documents related to the Referral Back Program and pharmaceutical services, it is shown that Referral Back Program pharmacies contribute to improving access to medicines, maintaining therapy continuity, and supporting healthcare service efficiency for chronic disease patients. However, its implementation still faces various challenges, such as limited drug availability, less-than-optimal coordination between healthcare facilities, administrative and claims issues, and limited human resources. Strategies to overcome these challenges include utilizing an integrated information system, strengthening drug supply management, improving pharmaceutical staff competence, and enhancing interprofessional collaboration. By optimizing these aspects, Referral Back Program pharmacies can increasingly play a role in improving the quality of healthcare services and supporting the sustainability of the National Health Insurance program in Indonesia.
