Abstract

Hand hygiene is the most effective measure in preventing Healthcare-Associated Infections (HAIs), yet healthcare workers' compliance in Indonesia remains below the Ministry of Health standard (≥85%). Head nurse motivation is expected to improve nurses' compliance with hand hygiene. Objective: To analyze the effect of head nurse motivation on nurses' compliance with hand hygiene at Pakuhaji Regional General Hospital, Tangerang Regency Method: This quantitative study used a quasi-experimental, non-equivalent pretest-posttest control group design. The sample comprised 44 staff nurses (total sampling of the accessible population across four wards), allocated into an experimental group (n=22) and a control group (n=22) through ward-level (cluster) randomization using the SPIN application. The experimental group received structured motivation from the head nurse (briefing, education, five-moments reinforcement, and feedback) twice weekly for one week; the control group received no intervention. Compliance was measured using the WHO Five Moments checklist before and after the study period and analyzed using the Mann-Whitney U test. Results: Before the intervention, most respondents in both groups were in the moderate category (75–84%). After the intervention, the experimental group improved significantly, with 17 respondents (77.3%) reaching the good category (≥85%), while the control group remained moderate (40.9%). The Mann-Whitney U test showed a significant difference (p=0.003). Conclusion: There is a significant effect of head nurse motivation on nurses' compliance with hand hygiene at Pakuhaji Regional General Hospital, Tangerang Regency. The structured motivation intervention through education, guidance, positive reinforcement, and feedback proved effective in improving nurses' compliance with the WHO Five Moments.
