The management of medical records at Belida Darat Community Health Center is still carried out manually, resulting in several challenges, including slow healthcare services, difficulties in retrieving patient data, and the risk of data loss or damage. This study aims to develop an Android-based patient medical record application to improve the efficiency of healthcare service data management. The system was developed using the Rapid Application Development (RAD) method, which consists of requirements planning, user design, construction, and implementation stages. The research data were obtained through observation, interviews, and documentation, while the system functionality was evaluated using Black Box Testing. The results show that the developed application integrates patient registration, medical record management, examination data management, and report generation into a single Android-based system, making the data management process more effective and organized. In addition, the results of Black Box Testing indicate that all major system functions operate according to the predetermined functional requirements and are considered valid. Therefore, the developed application can be used to support medical record management and contribute to the digital transformation of healthcare services at Belida Darat Community Health Center.
